These are quite generic but concerning signs. The list of differentials is very long in this case, so I would recommend a logic and radical approach. Begin by submitting a water sample for quality testing. Most pet stores will do this for free or a nominal fee. Clinical examination should always include a water quality check, even if you have done so already. Water testing should be performed with a quality test kit, measuring NH4, NO2 and NO3. High ammonia levels, PH imbalances may be responsible for these signs. Oxygen concentration and quality of the filters should be determined as well. If water quality is good and there are no major imbalances, your fish should get examined to look for any sign of bacterial/fungal infection and eventually water should be treated. What you have appears to be an aquatic/semi-aquatic species of turtle. Basic requirements include a tank of at least 10 gallons initially, a water heater, a water filter, a basking area out of the water, a heat lamp and a UVB light source.

Salt water aquarium requires a great deal of regular maintenance to keep the fish alive. This can be helped by adding live rock (similar to live coral) to the aquarium. Thanks for reaching out about your tank. Long plastic plants that extend from the bottom, all the way to the top can providing hiding spaces to decrease any territoriality or aggression. Live plants might be susceptible to the snails, depending on their type. Good luck.

Thanks for reaching out about Candoris and including photos. A veterinary visit is recommended as systemic antibiotics needed. Medications added to the water are not absorbed systemically and will not help with internal infections and will likely affect the biological filter. Check and correct water quality as pristine conditions needed to heal. Adding 1 gram non-iodized salt (aquariums salt) per 1 Liter, to decrease stress and osmotic load on the fish. Good luck.
