The bowl looks definitively a bit small for the two fishes. To understand exactly what is going on you need to approach this problem radically but logically. Begin by submitting a water sample for quality testing. Most pet stores will do this for free or a nominal fee. Clinical examination should always include a water quality check, even if you have done so already. Water testing should be performed with a quality test kit, measuring NH4, NO2 and NO3. High ammonia levels, PH imbalances may be responsible for these signs. Oxygen concentration and quality of the filters should be determined as well. If water quality is good and there are no major imbalances, your fish should get examined to look for any sign of bacterial/fungal infection and eventually water should be treated. Thanks for reaching out about you fish. It is believed to be a Comet goldfish. Water quality and nutrition is the key to success. Testing water parameter for pH, ammonia, nitrite and nitrate will give you guidance for a healthy environment. Purchased food should be rotated out every 3 months as vitamin C and other nutrients degrade over time. Trying to maintain water temperature can be challenging with out a heater, but most fresh water fish tolerate some changes. Good luck.

The small size will limit the species of fish as well as the number. White Cloud Minnows or Guppies would do fine in a small tank. You could keep no more than 2 or 3 fish total. If you are looking for company for your Betta, the same species can be kept. Other options include Otocinclus (Otos), Corys, Platys or Harlequin Rasboras. Again, no more than one or two in addition to the Betta. Water changes for any tank are simple. Change no more than 30% total volume at a time. Use a water conditioner to remove harmful chemicals and make sure the water is temperature matched to prevent stress or shock. Rinse the filter in tank water, not tap water, to preserve beneficial bacteria. Vacuum the substrate every couple of weeks.

For small bodied fish (tetras, minnows, guppies, etc) the general "one inch of fish per gallon" rule will likely be fine. For larger bodied fish (goldfish, plecos, Oscars, etc), it should be closer to one inch of fish per 3 gallons of water. Always use the full grown adult size when deciding how many fish to keep. Take into account schooling fish as well which should be kept on groups of at least 5-6 fish.

Java Moss, Java Fern, Anubias and Amazon Swords are among the easiest live plants to care for and cultivate in an aquarium. You can use aquarium soil or aquarium sand depending on the species of plant(s) chosen. When medicating fish, it is best to remove them to a hospital or quarantine tank. This will spare live plants as well as beneficial bacterial populations, especially if an antibiotic is used. Medicated food is also an option.
