At five weeks you can start transitioning to a puppy food that is moistened with some KMR or other replacer milk, or look for puppy mousse (Royal Canin and a few other companies carry it) that is designed for transition from milk to solid foods. Yes, I suggest you get him a probiotic to add to the food. Use the probiotic with the chicken and rice (no puppy food) for about 4 days and then go back to trying to transition him back to the puppy food. I like this probiotic. I think it is the best. Sudden diet change, GI parasites, eating food that irritates their stomach (such as the hot dogs), or something more serious like parvo or distemper virus. diarrhea and get the appropriate treatment. If his stool is clear of any parasitic, bacterial or viral infection, then I would consider a new diet. I would consider a sensitive stomach formula such as Purina ProPlan Sensitive Skin and Stomach.
