My dog can't urinate. Is it kidney failure or a bladder blockage?

I would have her evaluated just to be safe. Since she is in kidney failure you want to make sure that she continues to be able to produce urine. If you notice she keeps uncomfortable and is straining to urinate with nothing being produced, she should be seen by an emergency veterinary hospital as she may have a urinary blockage. The two main options are a urine infection or a urinary stone, from the symptoms described I would say that a stone would make more sense which means it is more urgent. Yes, I would definitely take Kahlua into an ER vet. I would be very concern that if she is trying to urinate and nothing is happening that she may have bladder stones. Is she trying to urinate but can’t? If so this could be an emergency and she should be seen immediately. It is also possible that she is dehydrated and doesn’t have much urine to produce. It is difficult to tell anything from the picture of the x-ray however, the urinalysis shows that there is a urinary tract infection which could account for the difficulty urinating. I am also concerned about the high levels of protein in the urine and a urine protein:creatinine ratio should be requested to assess kidney function. It is a good thing that the CRT is decreasing, however in order to properly evaluate kidney function the BUN levels are needed as well. Straining to urinate could be a medical emergency, and I would recommend having Blacky examined by a vet ASAP to determine if she has a urinary blockage. The vet may need to do an x-ray or ultrasound to look closely at the bladder.
