My puppy pees inside after neuter. Is it potty regression or UTI?

Yes it can be. Sirius had anesthesia, a surgical procedure and medications for pain. His routine was changed and his eating and drinking also. Most likely they gave him fluids during the procedure that can cause increased urination. Please go back to his housebreaking training. I recommend keeping him in a crate when you’re not home, and taking him out frequently for potty breaks. Reward him and praise him when he goes outside. When you’re home keep Sirius on a leash with you so he’ll ask when he needs to go, and he can’t sneak off somewhere and mess in the house. Inappropriate urination occurs when a dog regresses to urinating inside the house. This behavior is more common in male dogs that have not been neutered, although it can occur in non-neutered dogs and cats. The first thing to rule out is inappropriate urination that occurs when there is an underlying health problem which is causing a dog to urinate inside the house. It may have lost control of the muscles in its bladder or sphincter, or there may be an infection (UTI) or other condition as drinking too much (seen in many medical conditions) that is making it difficult for the dog to hold it in long enough to get outside. When a medical caused has been ruled out we can think about the second type of inappropriate urination which is behavioral (such as marking). He could have some inflammation from the neuter or a brief lapse in house training from the stress of the surgery and anesthesia. He could also have a urinary tract infection. I recommend calling your Vet Monday and see if you can drop off a urine sample. They can test the urine for infection. If he has a urinary tract infection then they can prescribe antibiotics. If hes doesn't have an infection ask if they can prescribe and anti-inflammatory, if he isn't on one already from his neuter, in cause it's just some inflammation. It has to do with the drop in hormone levels after the surgery and that has some effect on bladder control. Fortunately, this can be treated with medication if the accidents continue and do not get better. You may wan to visit with your veterinarian soon regarding medication if he does not improve soon. He is so cute! If you are finding that he is urinating while he is sleeping then this definitely could be incontinence. I would talk to your vet about trying Proin (aka phenylpropanolamine) to see if this helps. This helps to tighten the urethral sphincter to prevent incontinence. It is used most commonly on female dogs but I have had some success using it in males. If you are seeing puddles throughout the house, he could have a urinary tract infection. I would have your vet do a urinalysis on him to see if there is any bacteria present. Your vet will be able to prescribe him the proper antibiotics to help him feel better. I hope this helps!

The medications may have unsettled his stomach, i recommend contacting your vets office to discuss stopping them early. Confining him to one room at a time with frequent leash walks may speed return to a normal routine

The anaesthetic can stay in the system up to 24 hours which could still be effecting him. Alternatively he could be uncomfortable down there following the procedure. I wouldn't worry about it as yet if he seems bright but I would certainly get it checked out if it continues for a few days. Let him out often to try and avoid accidents.
