How to heal a dog's muzzle sore that keeps reopening from rubbing?

Place an e-collar (cone) on him to prevent pawing or rubbing. It should extend at least two inches beyond the tip of the nose. This may be enough to allow healing. If not, have him examined by your veterinarian.

Lesions in this location can be a little tricky to treat with topical medications because most dogs are able to lick them off, at least to some degree. You can try to clean this area with an antiseptic chlorhexidine cleanser twice a day (Hibiclens is one brand), but take care to rinse it well and do not let Bella lick it while you are cleaning. Breaking open a vitamin E capsule and rubbing it on this area may be mildly helpful, but it won't hurt if she licks a little off. As Jessica mentioned, one of the most important things to do is to keep an e-collar on Bella to prevent her from rubbing at this area further. If this lesion persists for another week or worsens at any point, you'll need to take her to a vet for evaluation. I'd recommend getting an Elizabethan Collar (aka "The Cone of Shame") and then apply some triple antibiotic on the area twice daily. Keep using the Cone until the area has healed. You can get Elizabethan collars at your vet and some pet stores. Good luck!

If you have already tried topical therapy without the lesions healing completely then she should probably be examined by a vet for treatment. She may need an oral antibiotic and anti-inflammatory to treat. Also, keep the muzzle off as it will probably prevent healing.

I would clean the area, but do not apply any ointment. Oftentimes this is irritating and may make them more likely to rub or scratch more. An Ecollar may prevent him from causing any more trauma, but doesn't treat the underlying cause. If you are unsure why he is rubbing, a vet visit may help to determine the cause of his itchiness. He may have mouth pain that is causing him to rub, or allergies making his face itchy. If the underlying cause isn't found and fixed, he is lilkely to keep rubbing and may cause more sores.

Sounds like Misty is feeling pretty rough! Just under the nose is a hard place to stop licking, as you can't use an Elizabethan (cone) collar to stop her from doing so. You may need to try to distract misty with a chew toy instead that she can put her energy into to help prevent her from licking the spot. if it is very raw and sore, you may also want to have your vet take a look to make sure there isn't an infection or other irritation that should be treated. Your vet may also be able to recommend a no-lick spray or bandage that may be suitable around the nose, as well. Avoid putting any home remedies on the spot, as it may just increase irritation which will increase licking and stop the area from healing.
