Cat got vaccine, now has severe diarrhea and vomiting. What's wrong?

Differentials are many, from vaccine side effects to any gastrointestinal diseases (infection/inflammations often dietary related) to any disease that can spread systemically (causing lethargy for example) like infection, and anything that can cause pain/discomfort. The approach must be logic, starting from offering different kind of food. A check up is needed if inappetence lasts for more than 24hours. Typical vaccine reaction is a slight temperature, soreness, vomiting and very lethargic. It sounds like Minnie might of had a mild vaccine reactions if he symptoms started right after the vaccine. However if she was vaccinated days before this started it is unlikely they are related. I would recommend taking her back to the vet for a good exam to make sure she does not have something more serious going on like a virus or infection. It would be hard to say if the vaccine is related without knowing what vaccine was given and how Leo has reacted in the past. Sudden behavioral or health changes are always concerning, so I would definitely bring it up with your vet and have Leo come in for an exam. Accidents can sometimes indicate underlying pain or illness, so ruling out more serious issues is best. I would also let your vet know about the lump that developed so they can monitor it or change how any future vaccine are given to Leo to help minimize reactions.

It is not a typical reaction to the feline distemper vaccine. If he is otherwise acting fine then you could withhold food for 12 hours and see if the vomiting and diarrhea resolves. If not, he should be rechecked by your veterinarian to determine the cause and treat. Your veterinarian can prescribe medications to help control the vomiting and diarrhea. I am so sorry that your cat Milo is ill following vaccines. These are classic signs for a vaccine reaction, and therefore I recommend having him seen by your vet right away. If you vet is currently not open, then I would consider taking him to your ER vet to make sure he is stable and ok. He will likely get medications (antihistamine +/- steroids) to help combat the reaction. I do hope that your pet Milo is able to make a full and speedy recovery. I would notify your vet about this as soon as you can so that precautions can be taken next year when he is due for his vaccine boosters to help prevent this from happening again.
