Why is my Shih Tzu vomiting yellow foam despite vet clearance?

It sounds as Schafer is suffering from a gastrointestinal condition, possibly an infection. it could also be due to ingestion of a foreign body material. i would recommend taking him to the vet the first chance you get, he will need to be checked first, if the vet thinks it's a simple gastrointestinal infection he will need an anti vomiting injection, antibiotics and a special food for a few days in order to get better.

I recommend to have some diagnostics. If your vet seems unwilling to do this, I recommend to get a second opinion. Chronic vomiting is not normal and can indicate many things including an ulcer in the stomach, a foreign object in the stomach, a tumor in the stomach, GI parasites, inflammatory bowel disease or even something like kidney disease. Once I have ruled out other concerns such as parasites and blood work to rule out other concerns I usually dispense a medication called metoclopramide. This stimulates the GI tract to keep moving which usually decreases bilious vomiting. I am usually able to use once daily, but it can be used up to three times daily. The amount of exercise you are currently giving Hugo should be just fine and is not in any means excessive. The vomiting could be due to the food so I would consider switching his diet altogether and see if this helps. Many cases (roughly 65%) of vomiting / diarrhea can be directly related to the diet or an imbalance of the normal intestinal bacteria. If Hugo is otherwise healthy but vomiting, I would recommend changing his food to something such as Science Diet or Royal Canin and feed this for 4 weeks. If he continues to vomit or worsens, I would have a veterinarian examine him so that the most appropriate diagnostic and treatment recommendations can be made. I hope this helps! It sounds possible that Magi may have acid reflux disease. I would inquire with your vet about trying a short course of antacids on her. If that has already been tried, then I would suggest doing some bloodwork to check for any organ issues. And if that again is normal, then a food trial should be done to treat for suspected inflammatory bowel disease. Some good food options here are going to be easily digestible foods such as Royal Canin hypoallergenic HP, Hill's Selected protein PR, or Hill's Z/D Ultra.
