How to prevent a Shih Tzu from vomiting bile on an empty stomach?

It sounds as Shawndee is suffering from a gastrointestinal condition, possibly an infection. it could also be due to ingestion of a foreign body material. i would recommend taking her to the vet the first chance you get, she will need to be checked first, if the vet thinks it's a simple gastrointestinal infection she will need an anti vomiting injection, antibiotics and a special food for a few days in order to get better. if the vet thinks it is more serious she will need a blood test and possibly an ultrasound in order to find out what is wrong.

I would ask your vet if they think Ziko may have inflammatory bowel disease. This disease can cause ongoing vomiting and/or diarrhea. The treatment can range from a special hydrolyzed protein diet (your vet likely carries one in their office) to medications that decrease inflammation in the GI tract such as metronidazole, prednisone, budesonide, cyclosporine). Definitive diagnosis actually require a biopsy but sometimes we will treat if we are suspicious and then monitor response to treatment. I think it is worth discussing with your vet if this is a possibility with Ziko.

I recommend to have some diagnostics. If your vet seems unwilling to do this, I recommend to get a second opinion. Chronic vomiting is not normal and can indicate many things including an ulcer in the stomach, a foreign object in the stomach, a tumor in the stomach, GI parasites, inflammatory bowel disease or even something like kidney disease. Dogs generally vomit bile when they have an empty stomach. I recommend feeding small meals several times daily to reduce the vomiting. Or you can give a treat or two just before bed and first thing in the morning. Generally this is nothing to be overly concerned about. Very small and very young dogs can quickly get dehydrated and develop low blood sugar if they have vomiting and diarrhea, so I would recommend taking Tinny Bubbles to the veterinarian as soon as possible to have her evaluated. If she is dehydrated your vet can give her fluids under her skin. They can also give her medication to stop the vomiting and diarrhea. We will sometimes recommend giving oral electrolyte supplementation such a Pedialyte at home to prevent dehydration with diarrhea, but if Tinny Bubbles is also vomiting, then she will likely just vomit it up, so she should see her veterinarian. Honestly we rarely have to put these dogs on a special diet but if I had to pick a diet I would say high fiber which may help with GI motility. A prescription diet from your vet like Hill's w/d would be best. I would use the canned version. I am a bit worried that the diagnosis is not correct. There are many other reasons why a dog could be vomiting bile (parasites, partial GI obstruction, GI motility disorder, cancer, endocrine disorder, food intolerance/allergy, etc). Did he have any blood testing done? X-rays? I think you have already done what should be tried for simple bilious vomiting syndrome. Since he hans't improved and is actually worse, I strongly recommend you consult with your vet about more testing. I am also concerned as this is a diagnosis usually seen in older dogs; Earl is young to have this issue.
