My dog still has worm symptoms after deworming. Vet or wait?

If he vomited 5 days after the deworming the vomiting is not likely to be related to the deworming. It sounds as Smokey t is suffering from a gastrointestinal condition, possibly an infection. it could also be due to ingestion of a foreign body material. i would recommend taking him to the vet the first chance you get, he will need to be checked first, if the vet thinks it's a simple gastrointestinal infection he will need an anti vomiting injection, antibiotics and a special food for a few days in order to get better.

He could still vomit worms - it can take a few days and usually two doses to take care of most roundworms. There are some types of stomach worms that would require a different dewormer so I'd recommend seeing your veterinarian if Roman continues to vomit. They can test Roman's stool to help determine what type of worms are present, and it might also help to reserve a worm if he vomits another up.

It is not uncommon for dogs to vomit after medication and generally if it is more then 2 hours after the medication should have worked. As the food was undigested it may not have been absorbed as well as anticipated and it may be worth applying a different type of spot on medication, especially if you are in a heartworm affected area. I would not deworm him when he is not feeling well unless you have a specific diagnosis that he needs the dewormer (example: a fecal test was run and you know he has roundworms). I would wait until he is feeling better.

Poor pups. While a heavy parasite load can cause severe GI upset, and anemia which can cause a pup to become extremely lethargic, have bloody diarrhea, etc, I would also want to rule out underlying illnesses as a possibility. It would be best to bring them to a vet ASAP especially if they are lethargic or unable to keep any food or water down as puppies can quickly become very dehydrated. They may also need supportive care such as an IV or injected medications if they are very ill from the parasites or other causes. You can try continuing to offer water, or rubbing a little honey on their gums for energy until you can get to the vet, however due to the severity of symptoms it would be best to seek emergency care.

I would recommend letting your veterinarian know that Bernie vomited a roundworm. It is very common for puppies to have roundworms and the dewormer is most likely killing them off. I would recommend depending on what type of dewormer to deworm again in 2 weeks or change the dewormer. Sometimes it takes a second round of deworming puppies.
