My 2-year-old Shih Tzu won't eat, only drinks water. What's wrong?

She can have a few different things going on. She can have a urinary tract infection or she may have an upset stomach from eating human food. She sounds pretty sick, and I'm concerned she isn't eating or drinking, having diarrhea, and has now vomited. If she isn't fully vaccinated, parvo could be causing her symptoms. Cookie is quite the cutie! I'm sorry she's feeling a bit off. With those very general symptoms, I would recommend keeping an eye on her and taking her to the vet tomorrow if she doesn't go back to normal. If she started vomiting, has white gums, doesn't want to move around at all, or seems uncomfortable, then I would recommend taking her to an emergency clinic for evaluation. Being such a young puppy, it will be easy for his blood sugar to drop too low from not eating. I recommend placing some karo or maple syrup on his gums and seeking care I hope this information helps!

If Bella ate dinner last night and again at midnight, it is possible that she is not hungry. However, if she continues to not eat through the remainder of the the day, I would recommend you take her to the veterinarian because this could be a sign of illness. If you notice any lethargy, vomiting or diarrhea, you should take her to the veterinarian right away.

This depends on a lot of factors including if she is otherwise exhibiting any symptoms of sickness, if she is eating canned food which already has water in it, how much water she is actually having, among other things. If she is not drinking anything especially if having other symptoms such as vomiting, diarrhea, loss of appetite and/or lethargy, she should be evaluated as soon as possible to prevent dehydration and to investigate the cause. If she is otherwise doing well and is eating canned food, it would be ok to monitor her for another 12-24 hours and if not improved to have her evaluated by a veterinarian then.
