It is not a good idea to try and apply ear mediccine while she is asleep. I suggest you ask your vet whether a differen medication such as Osurnia, which can be applied in the vet's office once and is good for 7 days is not an alternative to struggling with your dog. If not, you may need to put a muzzle on your dog every time you intend to apply the medication.

The main reason infections do not clear is they are not treated for long enough or it may have not been the best medicine to treat it due to resistance. Ideally she needs to be rechecked to see if a culture is needed or oral medicine help. You can try to get an ear cleaner to help keep it clean. IF the ear persistently itch then you may want to consider a food allergy

It does sound like allergies. I would not use the Posatex unless your vet confirms an active infection. Doing this can create an environment where antibiotic-resistant bacteria can develop, which can make treatment really difficult. You can use an ear cleaner and try an antihistamine like Claritin or Zyrtec.

It may take several days to see improvement after starting medications. If your dog is extremely uncomfortable due to inflammation in the ear, you may ask your vet if an anti-inflammatory drug such as a steroid or NSAID can be prescribed to help with the discomfort until the ear antibiotic kicks in.
