I am sorry but I cannot tell by the picture exactly what that is. It could be a placental sac that has an infection. If you have not been to the vet to confirm pregnancy I would recommend doing that. It is a good idea to know how many kittens are present and also that mom is healthy and can deliver than normally. Kitty should be nesting an showing signs of constractions. Ususally you will see her sides contract. If you are not seeing this is unlikely she is in labor. However taking her to a vet would let you know for sure.

She could have a uterine infection so she should be examined by a vet to determine the cause. Your vet can take an x-ray and run lab-work to help diagnose and then determine the best way to treat. She may need an antibiotic or need to be spayed.
