My cat's lung fluid and fever returned after treatment. What now?

The fact that she responded indicates it was at least in part sensitive to the treatment. It is possible that there was a mixed infection with other viruses especially herpes or inflammation which has not been controlled. Further investigation and testing would be recommended in order to determine all of the factors involved

It may need time to clear but it is also odd that he is still having problems a month later. I worry that this could be a more severe form of pneumonia or FIP. I would have x-rays repeated on a day when he is not filling well to assess his lungs. You may need to consider advanced diagnostics like an BAL to obtain a culture.

You could ask them if they work with a cardiologist. If she hasn’t yet had an echocardiogram (performed by a cardiologist) that will probably give you the best information. Hello, I'm sorry to hear this about Douglas. I think its a good idea that he is hospitalized with these signs of fever and noisy lungs. Persian breeds are brachycephalic, meaning they have a shortened nose and anatomically altered respiratory tract. This does make them more susceptible overall to respiratory infections like pneumonia. Some pneumonias can be viral induced (and then quickly get secondary bacterial infections which is why antibiotics are needed) and if that's the case, he should have at least some virus protection from his vaccines to reduce the disease severity compared to if he was unvaccinated. Many young cats will have respiratory infections and recover to live full, happy lives. Some of these cats will have chronic respiratory signs and disease over their lifetime afterwards, but this is often manageable and treatable. I hope this is helpful. I'm sorry to hear that Mittens has not been doing well. I agree that Mittens' chest x-rays appear to be progressing over the past 2 weeks. At this point, either pneumonia is less likely or the bacteria causing a possible pneumonia are resistant to her current antibiotic therapy. Cancer is absolutely a suspicion due to the x-ray progression. Consider asking your vets if they have considered performing either endotracheal or transtracheal washes on Mittens, which would be able to obtain a sample of fluid or cells in the lungs. These samples could be analyzed for cytology (which may be able to diagnose cancer) or culture (to determine if bacteria is present and an appropriate antibiotic). There is a chance that Mittens may not be stable enough to undergo sedation for this procedure, but your vets will be able to examine her to determine if the risks are worth the benefits. Poor guy. I would have him re-evaluated in this case. It sounds like he may need another round of antibiotics or the same antibiotic given for a slightly longer period of time. It is tough sometimes with kitties with URI's, because without culture we are hoping that our broad spectrum antibiotic will clear up the infection entirely. You also have the option of asking for your vet to do a culture, so that we know exactly which antibiotic will clear things up for Cairo. Your other cats immune system is likely better than Cario's, which is why he/she isn't affected. I would also consider keeping Cairo on anti-viral supplements (Lysine powder) long term to help boost his immune system to hopefully decrease the amount of infections in the future. Congested lungs means there is a fluid build up in them. I'm sorry the vet wasn't very talkative to you. I hope Moon does feel better soon, but if he isn't showing any signs of improvement in the next 24-48 hours, take him back into the vet. Best of luck.
