How do I get a vaccine record for a shot I gave my dog at home?

Pet's info: Dog | Mixed Breed Small (up to 22lb) | Male | unneutered | 9 months old | 3 lbs

I have a shorkie. I went to the CO-OP and got a 5 in 1 shot. I gave it to my dog. Now, how can I get a shot record for that?

You can keep a record of the vaccine yourself. Remove the sticker from the vial and place it, along with the date given, on a chart. You may even be able to find a vaccine record online or in the local Petco.
