My dog has masses in urine and foul smell. Is it a UTI or stones?

My next concern is are we dealing with some predisposing factor leading to these UTIs or is there a resistant bacteria in the urine that is not responding to the current rounds of medications? I would recommend ultrasounding the bladder and obtaining a sterile urine sample for culture and go from there. Have the crystals resolved yet? If there are still crystals in the urine, then these need to be resolved as the presence of crystals will lead to recurrent UTIs and symptoms. I hope this helps!

It is hard to look at x-rays through the PetCoach system, but I do not see any bladder stones. There does appear to be some gas in the bladder, which may have been inadvertently introduced through catheterization or could indicate the presence of a gas-producing bacteria. The prostate is significantly enlarged, to the point where it could constrict the urethra and cause straining to urinate. The urinalysis shows a possible infection, which could come from the bladder or prostate. I would not start urinary alkalizers without the certainty of oxalate stones. I am so sorry that Puffy is having this issue. I would recommend having an x-ray performed to rule out or in stones. If she has bladder stones then ideally she would have surgery. If she is about 10 years old I wouldn't be concerned about the surgery as long as her physical examination is good and her blood work is good. If she does not have bladder stones I would recommend getting a urine culture performed and potentially an ultrasound of her bladder. The urine culture will definitively rule out a urinary tract infection while the ultrasound will make sure there is no mass in her bladder. These signs might be compatible with a urinary tract infection/inflammation. Stress related (idiopathic type of inflammation), infection and presence of crystals are the most common causes of it. A clinical examination at your local veterinarian is essential to confirm these clinical suspicions and treat appropriately. Antibiotics may be needed. As long as he can urinate and is acting otherwise normally, it is likely not an emergency situation. Consider allowing your veterinarian to collect a sterile urine sample to submit for a culture and sensitivity panel. This may identify the bacteria responsible for Red's infection as well as the most effective antibiotic to use in treatment. Stones in his bladder or kidneys and masses may be ruled out with the ultrasound. He may need to be on long term or pulsed (alternating weeks or month on/off meds) antibiotic therapy.

Based on the report you provided it looks as if this is a normal population of bacteria that lives in the skin. However, if there are signs of infection, like inflammation and purulent exudate, then you may want to consider treating with an appropriate antibiotic. If this is the first time an infection has been present your vet will likely recommend an empirical treatment with antibiotics. However, you could also culture the exidate to determine what organism specifically are growing and what antibiotic they are susceptible to. It may be that Goofy has a bladder stone or has a tumor in his bladder that is an underlying issue. If a bacterial culture hasn't yet been done, that should be considered as well to determine definitively whether or not there is an active infection. Talk to your vet about an X-ray or an abdominal ultrasound to further investigate. While the ALT and globulin are slightly high, they aren't terribly high out of the normal range. Its reasonable to repeat these blood tests in 1-2 weeks and see if the values have normalized. Neither of those bloodwork findings are terribly alarming to me, especially if Goofy isn't showing any symptoms.
