My dog pees frequently, but nothing works. What's the next step?

Hi, thanks for using Petco Pet Education Center, formerly Petcoach! It becomes concerning when an older dog develops blood urine that is non-responsive to antibiotics. Having blood in the urine with out bacteria can mean a bladder stone or a bladder tumor. Tests I would consider next would be a urine culture, one belly X-ray to look for a bladder stone and full blood work. If the culture is negative and the X-ray doesn't show a stone then I would worry about a bladder tumor. An ultrasound of her bladder would help diagnose if she has a bladder tumor. Another option before doing more testing is to try a more powerful antibiotic for 3 weeks. But if the blood is still present with out the presence of bacteria then I would go forward. There may be some residual inflammation related to the stones. He may also have urinary crystals which may also be irritating. Dis uss anti-inflammatory or pain medi ation with your veterinary. Having either on board may make Watson more comfortable. A sterile urine sample should be collected and submitted for urinalysis, culture and sensitivity. If he has a UTI, the culture will confirm it. The sensitivity panel will determine the most effective antibiotic to use in treatment. It may be that any potential bacteria are not susceptible to amoxicillin.

Your veterinarian should perform a urinalysis and a urine culture and susceptibility (if they haven't already) to see if there is an infection present and to what particular antibiotics it is susceptible. Another possible cause of Gracie's signs could be urinary bladder stones, which can cause irritation of the bladder, leading to frequent urinations that are often red-tinged. Most bladder stones are visible on abdominal x-rays, which may be recommended by your veterinarian. Other stones require an abdominal ultrasound to see. An abdominal ultrasound would also be able to detect any other abnormalities in the bladder, for example, a tumor. Stress related (idiopathic type of inflammation) infection and presence of crystals are the most common causes of it. Another clinical examination at your local veterinarian is essential to confirm these clinical suspicions and treat appropriately. Ultrasound or x rays of the abdomen may be needed as there may be stones inside the bladder (which does not show up on urine analysis). Many times there is an underlying cause of recurrent UTI-like symptoms. Daisy likely needs additional diagnostics, like: Urine sediment exam: to look for crystals in the urine Abdominal x-rays: to look for bladder stones that cause inflammation and can create an environment for UTI Abdominal ultrasound: to look for possible tumors Urine culture: to look for resistant bacteria, this will also help your vet determine the best antibiotic to use. Other medications to consider are anti-inflammatories, prescription urinary diets, and bladder support supplements; you can discuss this with your veterinarian.

Unfortunately this is going to be quite the difficult case to manage due to such the resistance to antibiotics. In this case it would be worth trying a multi-antibiotic approach and you may need to resort to an antibiotic such as Amikacin, which can have adverse effects on the kidneys. Unfortunately, I see that Amikacin is also on the list of antibiotics that this is resistant to. There are more alternative methods of treatment such as ozone therapy in the bladder which may help kill the bacteria. I would discuss this further with your veterinarian as treatment will depend entirely on what the vet wants to use. Scotch may need to be admitted into the hospital for treatment for IV antibiotics as well. I hope this helps guide you in the right direction.
