Dog on Simparica for mange: What topical solutions for skin scratches?

While Simparica is licensed for flea and tick control it likely has a similar effect on mites (similar to Bravecto). Apoquel will not heal allergies, but it will relieve itching caused by allergies in many cases.

Wet dog and massage shampoo into entire coat. Allow shampoo to sit on him for 5 minutes then rinse thoroughly. Use 2x weekly for first two weeks and weekly thereafter. Once skin condition clears begin using on a regular basis to sustain a healthy coat (like once every 3-4 weeks). For use on dogs only. Prescription monthly Nexgard flea and tick preventative also kills mange and is approved for a 4 month old dog (monthly Simparica and Bravecto every 12 weeks also work off label to kill mange but are labeled for dogs 6 months and older). She can be just on monthly Nexgard without the ivermectin therapy but both are safe as well. I recommend inquiring with your vet about monthly Nexgard for Winter and seeing if she may have underlying allergies as well which can predispose dogs to getting mange. Prescription Apoquel pills or monthly Cytopoint allergy injections Injections are both very effective in treating itchiness from environmental allergies (typically characterized by paw licking, scratching, chronic anal gland issues, chronic ear infections). Usually a dog with sarcoptic mange is retested after 2 treatments and considered mite free after testing negative 2 months in a row though this protocol may vary depending on the veterinarian. Skin issues like you are describing can have a variety of causes, including skin parasites (fleas or mites), allergies, an infection, or an underlying medical issue (Cushings disease, diabetes). I would recommend he be evaluated by his regular veterinarian to determine the underlying cause for his skin lesions, have any necessary testing done (bloodwork, skin scrape, culture) and start on any treatment that he may need. It isn't uncommon for dogs going through mange therapy to initially get itchier as the mites die and cause inflammation. However secondary infections are also very common with mange. It is common for a dog to be on antibiotics and perhaps an anti-itch medication in addition while being treated for the mange. You could call the veterinarian that prescribed the mange treatment to discuss antibiotics and/or medication to help with his itching. Sometimes a topical shampoo helps but often oral medications are needed.
