My dog has indigestion. Pepcid AC isn't working. Is another dose safe?

Yes, you can give 10mg of Pepcid (famotidine). It should be given once a day at the same time each day. You can give it for 14 days straight and then stop. Hopefully the problem will have resolved by then.

In the long term, there is a thought the chronic famotidine use may decrease some nutrient absorption and lead to excessive gastric acid production with possible esophageal acid reflux if it is discontinued. However, most internal medicine specialists still preferentially recommend famotidine over other antacids (such as omeprazole/Prilosec) for chronic use as it is a bit more "mild" in its mechanism of action. I know you mentioned changing foods, but consider discussing a diet trial with a hydrolyzed "hypoallergenic" diet with Brody's vet. If he has a degree of a food allergy or inflammatory bowel disease causing his vomiting and skipping of meals, these hydrolyzed diets fed for 2-3 months can remove a lot of antigenic stimulation in the GI tract and potentially prevent a lot of GI upset.

Pepcid is safe to give daily, and I've used it for years in some cats with no problems. However, it's important to have an actual diagnosis before proceeding with this plan. Not all gastrointestinal problems are improved with Pepcid - it merely reduces the amount of acid in the stomach, so it doesn't help with all problems.

There are no reported problems administering these 2 medications together, although there is a warning about administering Sildenafil with a similar medication cimetidine ( Pepcid is famotidine ). So just to be sure, I would separate these 2 medications by at least 30 minutes, ideally would be 1-2 hours. Since famotidine can alter the ph of the gastric mucosa & thus potentially affect the absorption of other medications, I would give the sildenafil first. I'm not sure what the reasoning is on avoiding it with kidney disease in humans, but I can tell you that it's used extremely commonly in dogs with chronic kidney disease without any problems. It's a very safe drug in general, and can often be helpful for patients with this problem because kidney failure can cause excessive acid production in the stomach. The Pepcid helps with this.

Vomiting can happen in dogs after having anesthesia for being spayed, but it's good that she ate an hour later. If she ate and was able to keep the food down, then her stomach has likely settled. It's never recommended to give any human OTC medications without direct supervision from a vet, so I would not recommend giving Pepcid ac to her. If she does vomit again, you can withhold food for 12 hours, then feed her a bland diet of boiled white meat chicken (no bones) and white rice in small amounts several times a day. Then slowly add back in her normal diet.
