My dog licked his belly red and inflamed. Does he need antibiotics?

Pet's info: Dog | German Shepherd | Male | unneutered | 4 years and 2 months old

He started licking continuously at his abdomen and he does early in the morning when everyone is asleep so when I see it today I found this redness and rough inflamed skin with skin like hives and dry skin so today I apply topical creams and miconazole spray as well so by looking at attach pictures what do you say does he also needs and antibiotics course ?

I suspect Bruno needs antibiotics and anti-inflammatories. That lesions look very red. He shoudl also wear an elizabethan collar at all times to prevent licking which could make it worse. I hope this helps.
