Is my dog's skin infected? Antibiotics, cream, or vet? How long?

Hello. There are all different causes of skin eczema, but if your veterinarian put you on a combination of an antibiotic & anti-fungal medication, I would guess Bruno has a pretty serious skin problem. If he does have a fungal element to his skin disease, I just do not believe 5 days is long enough to do anything at all. Sometimes, these things need to be treated for 6-8 weeks before a cure is achieved. Does Bruno have any underlying organ dysfunction. I was not familiar with the trypsin , bromelain, rutoside preparation, but I found a paper where it was used to ameliorate the course of chronic renal failure in a rat model. Without knowing the exact diagnosis of Bruno's condition I cannot comment on your veterinarian's course of action. Perhaps he is just being cautious, but in my experience, most of these skin issues do take quite a bit more time to resolve than 5 days. Stay in close contact with your vet & follow all recommendations, but the actually drug amounts that were prescribes are reasonable to use over time. Poor Bruno. He should be on oral anti-fungal medication +/- topical antifungal cream or shampoo, depending on what areas of his body is affected. For really resistant infections often pets need extended time on the medication which can be anywhere from 2-6 weeks. I know it's tough to be compliant for that long, but it may be necessary to clear Bruno's infected skin completely and for good.

I would recommend that you have a veterinarian examine Bruno so that the most appropriate diagnostic and treatment recommendations can be made. This looks like a severe contact reaction or a severe skin allergy. There are many treatment options for allergies including Cytopoint injection, which helps in cases of atopic dermatitis. There are shampoos such as Douxo PS Calm shampoo and calming sprays that contain a steroid and topical anesthetic to provide relief. It looks like Bruno has a skin infection. Parasites like mites and fleas can cause irritation of the skin that can lead to bacterial infections. If he has not had skin scrapings performed to look for mites, that would be my recommendation. If there are no mites, and he has been treated for fleas, then perhaps the topical medications he is on are not enough, and he needs an oral antibiotic to control the infection. I hope this helps. You will first need to determine the type of infection. A recheck with your veterinarian would be recommended. Samples can be submitted for fungal culture as well as for bacterial culture and sensitivity. If it is a fungal infection, your vet can dispense an antifungal. Some of these infections can take weeks t months to resolve. If it is a bacterial infection, the sensitivity panel should determine the most effective antibiotic to use in treatment. Allergies should also be ruled out as an underlying cause. They can be frustrating to diagnose since they have multiple causes (food, environmental, parasites). Discuss allergy testing, food elimination trials, hydrolyzed diets and antihistamine use with your veterinarian. Legally, we are prohibited from recommending a medication in the absence of a physical exam. Any prescription medications will need to come from your vet.

There is a shampoo that can help somewhat but likely won't completely resolve this infection without prescription antibiotics through your vet.
