My dog ate a small amount of hair conditioner with the attached ingredient list. Should I be concerned?

Hello, thank you for using PetCoach. Sorry to hear this happened. Lord only knows why dogs eat these things!
I don't see anything particularly concerning or toxic in the ingredients however, it is best to call Pet Poison Helpline or SPCA Poison Control to be on the safe side. Even though we know the ingredients of the bar we don't know the actual amounts of each ingredient that were in the bar which makes it difficult to know if he even ingested a toxic amount of any particular ingredient.

I suspect that Rudy may just end up having some gastrointestinal signs like vomiting and diarrhea that should be self-limiting. Of course, there is no issue with taking him to the vet now if that makes you feel more comfortable.
