My senior cat has hyphema, kidney disease, and high blood pressure. How to help?

So sorry she is having these problems. It sounds like she should be re-examined by a veterinarian since her signs are not well managed. Your vet may need to add in a blood pressure medication and/or an anti-seizure medication, as well as adjust her thyroid medication. If your vet is not able to manage her disease than your could go to a different vet for a second opinion or even take her to a specialist for an evaluation. This sounds like she may have retinal detachment which can be a result of uncontrolled high blood pressure. The blindness may be permanent. I would absolutely take her in right away since she has other symptoms also and the sooner she gets treatment the more likely she will be able to recover. I'm sorry your cat has a retinal detachment. If the retinas remain detached for more than a day or two, the prognosis is poor for a return of normal vision. Therefore, the key to a successful outcome is rapid diagnosis and early administration of the proper medication to lower blood pressure. Given that you have already started medication, she may have a good prognosis for restoring her sight, but only the vet treating her can say for sure. Yes, please have her reevaluated. The blindness likely is caused by high blood pressure and the high blood pressure is likely caused by the kidney disease. Your cat will need to be evaluated and then put on blood pressure medication. Subcutaneous fluid giving should not stress her, if it is done right. I would guess that Ebony may have had retinal detachment secondary to high blood pressure. Kidney disease can cause increased blood pressure. If her retinas partially reattached she may have regained some vision.

I would recommend that Monet see a veterinarian. Sudden onset of blindness and dilated pupils can be secondary to high blood pressure in older cats. They can suffer from hyperthyroidism or a heart disease called hypertrophic cardiomyopathy. Both of these geriatric cat problems can cause these changes with her eyes. In order to stage renal failure, we need to know the urine specific gravity, blood pressure, and urine protein to creatinine ratio. Based on the values you provided, she has chronic kidney disease but I cannot stage it without further information. If the free T4 was elevated that is consistent with hyperthyroidism which often masks underlying kidney disease. Therefore, her kidney disease could be worse than we suspect based on the info you provided above. Realistically, she has two diseases and fortunately both are treatable. Ultimately, the prognosis depends on response to therapy. It is not uncommon for some patients to do well for a few years with appropriate therapy and kidney diet. Prior to treating her hyperthyroidism, her kidney disease should be treated (at home) with subcutaneous fluid therapy, antibiotics to cover for an underlying kidney infection, and vitamin B12 supplementation. After she feels better and begins eating consistently, discuss treatment for hyperthyroidism.
