Why is my 9-year-old cat still peeing outside the box with a UTI?

I am so sorry you are having to deal with this. Cats have weird brains, and often after a traumatic time trying to urinate, they start to think it is the litter box's fault and decide to pee elsewhere. First, you need to clean all urine with an enzymatic pet cleaner - this will break down the pheromones that you can't smell but he can. Second, even though he is cleared of the UTI, it is still possible that his bladder is inflamed. This syndrome in cats goes by many different names, including FLUTD or FIC. There are many different things you can try to help him out, including environmental enrichment, calming supplements, changing the litter and/or box to a completely different type, prescription anti-anxiety medications or anti-inflammatories. Cats can have chronic urinary tract infections, stones, bladder cancer, or any number of issues causing them to urinate outside the box even after treatment. Sometimes this is very frustrating to treat. I am concerned that she is pawing at her mouth, as this usually indicates severe dental disease or oral infection (stomatitis), both of which being very painful conditions. If she has not had a full oral examination, she needs this to address whatever is going on in the mouth. She probably needs to have her urine cultured as well if she seems to have an infection that is unresponsive to antibiotics.

One possibility is that your car has a UTI, this is more common in female cats but does occur in males. Another possibility is that your cat could have bladder stones. Stones are very irritating and cause discomfort especially when trying to urinate. A third possibility is that your cat has a stress condition called FLUTD (feline lower urinary tract disease). This condition is common in young male cats who live indoors. A stressful event in their environment leads to inflammation and the symptoms you are seeing. I would recommend having your cat seen by your veterinarian so that they can check for a UTI. They will likely collect a urine sample and look for signs of infection. Maybe, maybe not. In Kitty's case, it could mean that the CKD has gotten worse, that there is an infection or other urinary issue like stones, or that Kitty isn't feeling well/anxious. Thanks for using Petco Pet Education Center, formerly Petcoach! There could be a structural problem- meaning pouches in the bladder or abnormal placement of the ureters. The other concern would be stones in the bladder causing recurrent infections. I recommend taking her to a vet for an X-ray to look for stones. I would then talk to the vet about a prescription diet to help decrease the frequency of her infections. Yes, I would bring Pigeon into the vet. This can be a sign of a UTI or bladder stones, and the only way to know for sure is to have her checked out by a vet, do bloodwork, and a urinalysis. Because she was using the box previously and now isn't, that can also be another sign something is wrong. Give her vet a call now to make her an appointment. Until she sees the vet, feed her wet/canned food, add water to it, and make sure she drinks a lot of water to help flush the bladder. I hope this helps!
