Why does my cat keep getting UTIs despite clean litter and good food?

This is very abnormal for a cat this young to have these chronic issues. If your family vet is stumped then it would be best for your kitty to see a specialist. I would recommend asking for a referral to a veterinary internist. Internists have advanced training beyond a family vet and can diagnose and manage more complex cases such as this one. Any blood work performed? This is a crucial first step. Xrays of the spine? Have these UTIs been diagnosed and treated by running urine cultures? With a chronic history this is very important. Has any endocrine testing been performed? There are some congenital/developmental disorders that should be ruled out as well given the history. An abdominal ultrasound is important as well to assess for structural changes to organs. Unfortunately in order to help your kitty she needs more tests. Not all pet illnesses are easy to diagnose and some cases can be complex. If Taffy is predisposed to urinary problems / crystals, I would recommend considering a prescription diet such as Hills C/D or Royal Canin Urinary SO. I would also consider a cranberry supplement such as Paxon, which can be found online quite easily. Increasing water intake is also key. A recirculating water fountain and feeding a canned diet can help with this. I hope this helps! I recommend that she has a urine culture performed to ensure there is an infection present and the correct antibiotic is prescribed. If there is not an infection or if it has resolved, then she should have x-rays to look for bladder stones. If this is not the case, idiopathic cystitis is very common in cats. I cannot tell you for certain whether the spraying is associated with the current urinary issue or if it is a behavioral issue. If I had to guess, I would say it is likely related to his current medical situation. You need to talk to your vet about this as they have all his medical information that I do not have. I'm also assuming he has bladder crystals and not stones? If he has stones then he would need surgery to remove them and that may be why the urinary is bloody. You need to clarify that with your vet. Cats can have chronic urinary tract infections, stones, bladder cancer, or any number of issues causing them to urinate outside the box even after treatment. Sometimes this is very frustrating to treat. I am concerned that she is pawing at her mouth, as this usually indicates severe dental disease or oral infection (stomatitis), both of which being very painful conditions. If she has not had a full oral examination, she needs this to address whatever is going on in the mouth. She probably needs to have her urine cultured as well if she seems to have an infection that is unresponsive to antibiotics.

There are several diet formulations to help pets who are prone to urinary tract infections. These are prescription diets and need to be approved by your veterinarian but they are highly effective. Royal Canin makes a diet called Urinary SO that most pets like the taste of. Science diet and Purina also make their own formulas.
