My cat is licking privates and peeing outside the box. Is it a UTI?

If Barbie is uncomfortable or experiencing pain from the infection, the twitching could be secondary to the infection. These conditions are typically very painful and uncomfortable as there is a large amount of inflammation and pressure from being unable to void the bladder. There are a few reasons why Harley and Marley may be urinating outside their litter box. Certainly things like a urinary tract infection are possible. Crystals in the urine are another reason this could occur. I would recommend to make an appointment with their vet to have their urine tested for infection/crystals. Behavioral issues are another reason for not using the litter box. Changes in the type of litter, the litter box itself or the location of the box can all effect behavior. The litter box needs to be cleaned daily. In general, cats prefer a clay based clumping litter in a large, uncovered litter box. They also want the box in a quiet and private location. Cats do not want to be cornered in the box by other cats or dogs so make sure it feels safe for the kittens. Lastly, adding a second or even third litter box can help. I recommend that she has a urine culture performed to ensure there is an infection present and the correct antibiotic is prescribed. If there is not an infection or if it has resolved, then she should have x-rays to look for bladder stones. If this is not the case, idiopathic cystitis is very common in cats. Sometimes this is very frustrating to treat. I am concerned that she is pawing at her mouth, as this usually indicates severe dental disease or oral infection (stomatitis), both of which being very painful conditions. If she has not had a full oral examination, she needs this to address whatever is going on in the mouth. She probably needs to have her urine cultured as well if she seems to have an infection that is unresponsive to antibiotics.

If it is red-colored urine and Lily has been urinating more frequently lately, then it is most likely a bladder infection/inflammation, but could also be a kidney infection. If it appears like fresh blood at the end of the urine stream, then this could indicate a problem with the vagina, cervix, or could be coming from the rectum. Either way, it's best that you take Lily to a veterinarian for treatment soon. If she is going to the litter box and nothing is coming out within the last 24 hours, then this is an emergency and I recommend you take her to the vet immediately. They will be able to locate where the problem is and provide the most appropriate treatment. Either way, Lily will need a professional to diagnose this problem and to provide the most effective treatment. Without good treatment, she can have life-threatening complications.
