My cat has severe, painful ear infections. Why aren't drops working?

If medication did not help then I would recommend looking for ear mites under a microscope at your veterinarian's office. If this is negative I would then send out a culture to see what type of bacteria is growing since the medication that was previously prescribed may not be the right medication for that type of bacteria. Ear infections can be caused by bacterial, parasitic or fungal infection, or a combination of these, and can be associated with dietary or environmental allergies or sensitivities, immune system disorders, or a combination of the above. If the Surolan helped but the infection has returned, Melcho needs to see a vet to have an ear swab done. Your vet can look at the sample under the microscope and treat appropriately based on what is found. Trying different medications without testing to determine the cause is not effective, and can lead to bacterial resistance to medication. Your vet will likely need to examine the ear with a scope to see if the ear drum is intact, as some medications cannot be used safely if the eardrum is ruptured. They may also recommend a culture and sensitivity test of a swab from the ear to help figure out what specific bacteria could be present and what medications they are sensitive to. Ear infections can be very painful, and as you mentioned, can sometimes progress to include the middle or inner ear, where they can cause neurologic signs or permanent hearing loss. The discharge inside the ear needs to be cultured so you can know what is growing in the ear canal - what bacteria, yeast, etc. That way the vet will know what medication to use to get it to clear up. The ear may also need to flushed out under anesthesia. There must be an underlying cause. Cats typically don't get ear infections chronically like dogs. It is not likely to be a food allergy. More testing needs to be done.

If the antibiotics are helping, it's probably a middle ear infection indeed. a sample needs to be taken from the ear and sent for culture and sensitivity, then the best antibiotic can be chosen according to the result. the course of treatment must be longer next time, at least 4 weeks as these infections take a long time to resolve.

I would recommend taking grandma kitty back to the vet if you are getting this much discharge still. It appears the medication they gave you is an ear cleaner with an antifungal in it. It is possible she needs stronger antibiotic drops. Typically animals do not need oral antibiotics for outer ear infections just topical. I also would request they check for ear mites to make sure none are seen. Hope that helps

Hello, sorry to hear about Sugarplum. It sounds like she may have a persistent ear infection. She may need to have her ears cultured to make sure she is on appropriate antibiotics. In addition to a longer course of oral antibiotics, she may need to have antibiotic ear drops prescribed as well. Ear infections are relatively common in both cats and dogs. Some are more persistent than others. As you said, your vet took a look at the contents of the ear to determine what course of treatment might be required. Without a true bacterial / fungal culture, I could only speculate as to what the exact organism might be. The majority of ear medications, tresaderm included, are traditionally quite broad spectrum in an attempt to cover as many bugs as possible. The reason Fatboy is shaking his head is likely a combination of both the drops and the discomfort associated with the infection. If after six days of effective treatment, you are not seeing any improvement at all, you might want to call you vet and let him know. He may want to obtain a new sample to see if any progress is being made. If not, a true culture could be sent out to better target his therapy and change medications if needed be.
