My cat's ears are still wet and itchy after vet treatment.

I would recommend taking grandma kitty back to the vet if you are getting this much discharge still. It appears the medication they gave you is an ear cleaner with an antifungal in it. It is possible she needs stronger antibiotic drops. Typically animals do not need oral antibiotics for outer ear infections just topical. I also would request they check for ear mites to make sure none are seen. Hope that helps

The discharge inside the ear needs to be cultured so you can know what is growing in the ear canal - what bacteria, yeast, etc. That way the vet will know what medication to use to get it to clear up. The ear may also need to flushed out under anesthesia. There must be an underlying cause. Cats typically don't get ear infections chronically like dogs. It is not likely to be a food allergy. More testing needs to be done.

If the antibiotics are helping, it's probably a middle ear infection indeed. a sample needs to be taken from the ear and sent for culture and sensitivity, then the best antibiotic can be chosen according to the result. the course of treatment must be longer next time, at least 4 weeks as these infections take a long time to resolve.

Ear mites and underlying allergic disease (that can predispose to recurrent ear infections) are possible and should be ruled out. A physical and otoscopic examination of the ear canal are enough to rule them out. Bacterial swabs and culture can be used as well to rule out resistant infections. It looks like your kitty has a skin and ear infection. The medications he was on likely did not treat the problems. He needs a difference antibiotic for the skin and a medication that goes into the ear. A vet needs to determine what kind of ear infection that is before prescribing medications. Poor Spookette! It sounds like she may have a pretty bad ear infection. Ear infections can be due to mites, bacteria, or yeast in cats. I'd recommend scheduling an examination with a veterinarian so Spookette can have her ears checked out. A swab can be taken from the ears and evaluated under the microscope to determine if the cause is mites, bacteria, or yeast. She will likely need prescription ear medication to treat. Ear infections are relatively common in both cats and dogs. Some are more persistent than others. As you said, your vet took a look at the contents of the ear to determine what course of treatment might be required. Without a true bacterial / fungal culture, I could only speculate as to what the exact organism might be. The majority of ear medications, tresaderm included, are traditionally quite broad spectrum in an attempt to cover as many bugs as possible. The reason Fatboy is shaking his head is likely a combination of both the drops and the discomfort associated with the infection. If after six days of effective treatment, you are not seeing any improvement at all, you might want to call you vet and let him know. He may want to obtain a new sample to see if any progress is being made. If not, a true culture could be sent out to better target his therapy and change medications if needed be.
