Why is my female cat's belly big after heat if she's not pregnant?

Other signs of a pyometra in cats can include vomiting, lethargy, and a discharge from the vulva. If you are really scared she has a pyometra, my advice would be to bring her into a vet right away. A pyometra is fatal without treatment, and time is of the essence for the success of treating them. I see that Princess isn't spayed. I would be concerned that her accidents in the home and her distended belly is due to a pyometra. This is a life threatening infection of the uterus that causes it to become enlarged with pus. I would have Princess seen by a vet immediately. My recommendation would be to have a physical exam, blood tests, a urinalysis, and possibly x-rays to diagnose if she does have a pyometra. If she does, the recommended treatment is spaying her. Yes it is possible for a cat to get pregnant at the age of 6 months, sometimes even at 4 months. Her nipples are indeed very big and this could point to a pregnancy. If she is gaining circumference and has been for a while, and if she had access to outside when she was in heat, then it is very likely she met a tomcat. You can have her checked by your vet (xrays or ultrasound) to check for pregnancy and also for the number of kittens, which will help determine when she is done giving birth.
