Is river sand safe to use in a Betta fish tank? What are the risks?

You can, however it isn't as simple as collecting a few bucketsful and dumping them in your tank. You will need to properly clean the sand to remove impurities and parasites. This involves soaking the sand, water changes, more soaking amd more water changes. It may take several weeks. There are videos you can watch on the bigger video hosting sites. It is much easier, and safer for your fish, to buy sand that has been prepared already.

Creek water would not be recommended as you would likely need to heat it and the risk of waterborne pathogens (even in fast moving water) would be greatly increased. Ordinary tap water can be "aged" by leaving it out in a wide mouthed container for 24-48 hours prior to use. A water conditioner would still be recommended to minimize stress after water changes.

Anything you bring from outdoors will carry a small risk of exposure to things like intestinal parasites, but overall these risks are very small and natural sand should be just fine to use. Heating it in the oven and then letting it cool is a great way to minimize risk.

Yes, this is fine for a Betta tank. You should still use a small filter as well as a small submersible heater to maintain water temp. Bettas do best between 78 F and 80 F. If you choose to use tap water, use an appropriate water condition to remove harmful chemicals and heavy metals.

If the Betta tank is currently empty, I would suggest changing the gravel and cleaning it very well before putting another fish in it. That's terrific news! It is great to read of your frog's recovery. Most keepers will keep a bare bottom tank. This will prevent ingestion of substrate and makes finding food easier for the frogs. It also makes it easier to keep the tank clean. Sand would be fine. If they inadvertently ingest some, it should pass easily. If you decide on gravel or river rock, make sure it it large enough to prevent ingestion.
