Senior dog with sarcoma: Is surgery necessary if he's not in pain?

Pet's info: Dog | Mixed Breed Medium (23 - 60lb) | Male | 26 lbs

Vet thinks surgery would be difficult due to location and recommended an Oncologist. I'm going through radiation myself right now and don't want to do that to him He's 13. Report says PROBABLE Soft Tissue Sarcoma with mild mixed cell inflammation. From what I'm learning, it's pain free and he'll have 2-4 years without intervention. Do you think surgery looks like an option? I'm leaning towards just leaving it be. He's not in pain and still jumps and runs and feels good! He doesn't even lick it.

Hello, my name is Dr. Shines and I'm sorry to hear you and Nico are going through all of this. Your plate is definitely full.

The decision to pursue treatment for cancerous masses is a very personal one and as long as Nico is not allowed to suffer, there is not a right or wrong answer.

I agree surgery in that area will be hard if you are looking to keep the foot. It's already a big mass in an area that has very little extra skin (versus if it was some place like the shoulder where there's lots of extra skin).

Soft tissue sarcomas tend to be locally invasive and spread 'cancer fingers' into the surrounding tissue that are microscopic. When these masses are removed, you have to take aggressive margins and that's going to be really hard for Nico in this location. Due to this reason, amputation of the limb can be an option when otherwise removing the mass is unlikely. That's certainly a big decision to make.

The other concern with this mass is location. It can be easier to traumatize since it's on the foot. Also, it can out grow the blood supply and start to become a chronic wound (which is not good).

I always recommend to have a consultation with an oncologist. Even if you opt not to pursue treatment, you will know all of your options and make an informed decision.

Typical life expectancy for a medium sized dog is 14-16 years of age. It is valid to take his age into account when deciding what to do.

In the end, as long as you do not allow him to suffer or be in pain, you will not make a wrong decision. Opting to allow Nico to live out his days with this mass (as long as he is comfortable and happy) is a valid option. Opting for treatment is a valid option. You must do what feels right in your heart.

Gumbo definitely is at increased anesthetic risk (risk of adverse events during anesthesia and risk of poor recovery) at age 14 (7-8 years of age is considered geriatric), even with normal blood work (normal liver/kidney values). In addition those are extremely large masses and surgical removal could definitely affect anal tone (result in incontinence) assuming that anesthesia and recovery do go well. There is also a possibility that the type of cancer Gumbo has is metastatic and has already spread to the lungs for which the prognosis is poor (chest rads would be recommended prior to proceeding with surgery). I would recommend bringing Gumbo to have the anal glands checked regularly to make sure that they are empty. I also recommend keeping Gumbo as comfortable as possible as consulting with your veterinarian about treatment and humane endpoints if the masses become ulcerated.

Senior dog with growing tumors and new symptoms: Surgery or comfort?
The first question to be addressed is whether or not Zara is physically strong enough to withstand the surgery. She will need a full physical exam with blood work to determine this. Second, the type of tumor and its staging needs to be addressed. Are these masses localized, or have they spread to lymph nodes, lungs, brain, etc? Finally, what aftercare would Zara need? How debilitated would the procedure leave her? The ultimate question for you to consider is length of life vs quality of life. At her age, it may be best to let her live out the rest of her life in relative comfort, rather than in a lengthy--and possibly painful--recovery.

My large dog has bone cancer. Are there alternatives to euthanasia?
Hi, thanks for using Petco Pet Education Center, formerly Petcoach! I am so sorry about Chunk's diagnosis! This is a terrible situation! If Chunk is having such a hard time now, it will likely be the same if the rear leg was amputated. Amputation is just palliative unfortunately, meaning it buys you and the pet time. Most bone cancers have microscopically spread by the time they are diagnosed. If Chunk's tumor is a fibroscarcoma then it is slower to spread and he could get more good time if he loses weight and is able to learn how to get around with 3 legs. So, if you are not ready to give up or give in to the situation, ask to have a biopsy done of the tumor so you know what cancer you are dealing with. If it is a fibroscarcoma then amputate and deal with losing the weight afterwards. If it is osteosarcoma then I think putting him down is fair. I am again very sorry that this is happening to you and Chunk. He is too young!

My senior dog has a mass, biopsy inconclusive. Is more testing humane?
HI, thanks for using Petco Pet Education Center, formerly Petcoach! Poor Yogi! I am sorry you are going through this. It is hard to jump in to a case like this with only a CT image. It might be better for you to sign up for a consult with one of out Vets to have a one on one text conversation. My impression of the CT is that there is a mass in Yogi's chest, in the mediastinum. There is abnormal fluid accumulation likely from the mass and there are also suspicious nodules with in his lung tissue. Which I think is safe to assume is metastatic. If Yogi was my dog and he had a chest tumor with suspected metastatic disease I wouldn't pursue further diagnosistics or surgery as his prognosis is very poor. To put him through a painful surgery when it has spread seems unfair for him. If the "biopsy" was really an aspirate they tend to sometimes be non-diagnostic as most tumor cells do not exfoliate well in to a needle. A true tissue sample would be needed. I am not sure knowing which type of cancer he has will change how I would proceed. Another option is to see a Veterinary Specialist. I would recommend an Oncologist. Sitting down with all the tests and having a discussion may help you. You can also call your Vet and ask to sit and talk about the results with out Yogi present. Just so you know all that you can before making any decisions. I am again very sorry for you and Yogi. Let me know if I can help in any other way.

My old dog has nasal discharge. Is surgery worth it for suspected cancer?
Poor Spike! I know this is a tricky situation due to his age. Every dog handles this dofferently but the older they get the harder it is. I would consider his age and talk to your vet in best case scenario how many GOOD QUALITY months could you get out of him. If it is only projected to add a month to his life then I am not sure it is worth putting him through an extensive and painful surgery plus radiotherapy. On the hand, if it 6 more months, then it may be worth it. If you dont plan to do the radiotherapy and surgery alone wont extend his life then I personally wouldnt do surgery at his age. I would talk to your vet abot what can be done just to make him comfortable - pain meds, appetite stimulate, long term anti-fungals or antibiotics, etc. In seniors, I always like to consider how to give them a good QUALITY of life over QUANTITY. My old dog has skin tumors. How do I know when to say goodbye?
Thanks for using Petco Pet Education Center, formerly Petcoach! I only see cancer patients and as you can imagine, I see terminal cancer patients on almost a daily basis. First of all, sorry to hear about Timmie. Give her lots of love, treats and all the good stuff that she deserves and wants. Usually regardless of the cancer type, I recommend treating any symptoms if they occur. Meaning that if Timmie develops any decreased appetite or nausea or vomiting or diarrhea, these are treated with medications. Otherwise, if you not pursuing cancer therapy, there is really nothing else to do. This decision is hard to make, no question. However, what I tell my clients is to consider making this decision if they think that quality of life has been severely affected. This means when Timmie does not seem to be enjoying herself anymore. You could consider doing it if Timmie stops doing 2 out of her 3 favorite things (that could be eating, playing, running, etc). Best wishes for you and Timmie!

Is amputation right for my older large dog with bone cancer?
Unfortunately only your veterinarian can answer this question. In general amputation buys a few months time and considering your dog is already very old for a Belgian Shepherd, it may be more humane to give your dog palliative care than go through with the surgery. If your dog has arthritis in his hips, having only one hind leg may put serious strain on the arthritic hip and this will cause more pain. Discuss good pain management with your veterinarian. An alternative would be to go see a veterinary oncologist who is most experienced in assessing bone cancer cases and can give you good statistics as well as has most experience with good palliative care.
