Senior dog with growing tumors and new symptoms: Surgery or comfort?

I'm so sorry Zara is so sick! The best thing you can is continue to offer her food and water, keep her warm, and make her a comfortable as possible with a soft bed. I would evaluate her quality of life every day, and make a list of five things Zara loves to do. If she isn't doing at least two of them, then her quality of life has declined to a point where euthanasia should be considered. I would also speak to your vet about pain medications, because mammary cancer can be very painful to dogs. That would also help her be more comfortable.

Assuming Kara is in good health otherwise - which should be established with blood work and x-rays of the chest, to ensure that the tumor hasn't metastasized - then I would recommend proceeding with surgery. Mammary tumors can be quite invasive and aggressive, but depending on the grade surgery can be curative. Just because a dog is old, it doesn't necessarily mean they can't have anesthesia - you just have to take precautions, and find a vet who is capable of taking special care. I would also get a definitive diagnosis as to what the mass is before making any decisions, There are lots of benign lumps that grow all over dogs' bodies, and just because this one is in the mammary area, it isn't necessarily an aggressive cancer. A fine needle aspiration biopsy can usually tell you what these are.

Gumbo definitely is at increased anesthetic risk (risk of adverse events during anesthesia and risk of poor recovery) at age 14 (7-8 years of age is considered geriatric), even with normal blood work (normal liver/kidney values). In addition those are extremely large masses and surgical removal could definitely affect anal tone (result in incontinence) assuming that anesthesia and recovery do go well. There is also a possibility that the type of cancer Gumbo has is metastatic and has already spread to the lungs for which the prognosis is poor (chest rads would be recommended prior to proceeding with surgery). I would recommend bringing Gumbo to have the anal glands checked regularly to make sure that they are empty. I also recommend keeping Gumbo as comfortable as possible as consulting with your veterinarian about treatment and humane endpoints if the masses become ulcerated.

While it is impossible to tell exactly what type of tumor it is and give a prognosis following removal, it is likely that removing the tumor will make him more comfortable. Depending on the approach, the surgical site may be rather large and require some care and pain management at home. As long as Jake is otherwise healthy, surgery and anesthesia are relatively safe as well. I hope this helps and am happy to answer any other questions you have!

Eighteen years is quite old for a dog, so I understand why you may be hesitant to go through any procedures that may decrease her quality of life. At this age it's uncertain how much longer she will live (with or without the tumor) and it seems like she may have other problems,or possible spread of the tumor that would not give a favorable prognosis for recovering from surgery. I would definitely want to solve the problem of not eating before recommending surgery for the tumor. It was helpful to see the notes and lab work, but I would need to further evaluate her and discuss this more in order to give you some more helpful advice. I think it would be worthwhile to discuss this with your veterinarian again and ask what they would do if it were their dog. This may help you with your decision. Unfortunately, this is a difficult situation because most tumors that are cancerous and rupture are not going to heal on their own. A double mastectomy will be a very large and could be difficult recovery time for an 11 year old dog. Also, please know that even with surgery, you may not get another 3 years out of her simply because she is already at the top end of her life span. Also, there is always the risk that the cancer has already spread. Another option you may want to talk to your vet about is whether they can just do a palliative "debulking" of the mass that has ruptured. If there is a way to just resect the one tumor that has ruptured, this potentially could give her a good quality of life without doing a full double mastectomy. With the tumor being ruptured, I am afraid you are being forced to either do a debulking surgery, full mastectomy, or euthanasia. Leaving it alone isn't a good option because it won't likely heal on its own and if it isn't already, it will become infected.

This is an extremely difficult decision to make. I always try to look at a dog's quality of life to determine if they are still able to do most of the things they've always loved. If they are unable to do half of their favorite things, it is likely that their overall quality of life has declined and euthanasia may be warranted. Since your dog is currently eating, drinking, acting and playing normally, it does not sound as though her time has come. However, since one of her masses ruptured, there is a chance that it could become infected. Ideally, the mass would be surgically removed and biopsied. If surgery is not an option, she may need a course of antibiotics to prevent the onset of infection in the open lesion. If the open mass continues to bleed or begins to cause discomfort, you may need to decide if she should have surgery or euthanasia. If you are beginning to question if the time for euthanasia has arrived, consider discussing this with your vet to get their take since they have examined your dog and can provide some insight into her current quality of life.
