What's this painless lump on my senior dog's neck under her ear?

It looks like her skin is just very inflamed in that area from all the scratching. I suggest you take her to your vet for an exam. She will likely need some strong prescription medications to get this to clear up. The leg appears to have an elbow callus. Her veterinarian will determine if this is the case. If a mass is present on the ear, it may require surgical removal. Unfortunately yes, these are painful, and they are very difficult if not impossible to treat at home. Chunk needs pain relief to help with the severe inflammation in this area, and also antibiotics to clear up the infection. I'd really urge you to get this treated sooner rather than later, as it's intensely uncomfortable, and there's really not anything you can do at home to treat it.

This could be a sebaceous cyst that ruptured and is now secondarily infected or has an open wound on it that won't heal. I would recommend having a veterinarian take a look at this and have some additional testing performed on it to determine what it could be. With more information on what this could be, the most appropriate treatment can then be started. If you are unable to get to your vet right now, I would recommend using an ointment called zymox topical ointment to apply to the area.
