What's the safest place for my puppy to sleep: leash, bed, or crate?

Congrats on your new puppy! Lilly's crate size should be big enough that she can turn around and lie down in it, but prevent her from going to the bathroom in it. If the space isn't too big, she will not go to the bathroom in it because she won't want to lay down in her own mess. I wouldn't put the pee pee pads in there, as you don't want her to learn to go to the bathroom where she sleeps. Given that she is so young, I would wake up at least once or twice in the night to let her out to potty. If you put the crate next to your bed, she will whine and cry all night to be let out, and you won't get any sleep. I would recommend keeping the crate in another room that is quiet and dark.

I would crate him at night since you cannot watch him and he is so young. Once you know that you can completely trust him (potty and getting into stuff) then I would say you can let him out of the carte and sleep with you. This is a very common problem with puppies :) It sounds like the pen he is in is probably too large - it gives him too much space to work with, and makes it easy for him to wake up and find a spot to pee, rather than either holding it or whining to let you know he needs to go out. Instead of the pen, I would recommend having him sleep in a crate in your bedroom at night. Most puppies instinctively do not want to pee where they are sleeping, so in most cases this means that the pup will whine to wake you up if he needs to go out during the night. As he gets older and his bladder control improves, he should start to be able to hold it all night with no problems.

In my personal opinion, this is not a problem at all, i have never seen any negative effect on a puppy sleeping on it's owner's bed rather than in a crate. as long as you are comfortable with the idea that he is likely to always want to sleep on your bed in the future it's absolutely fine.
