Why isn't my puppy gaining weight and has yellow, unhealthy poop?

It could be Lady is suffering from intestinal worms causing her to have diarrhea and not put on weight. They can be common in puppies. I would have her checked out by a vet today or tomorrow, and bring in a sample of her feces with you for the vet to examine under a microscope. Poor Pip! She is so lovely. I'd take her back into the vet this weekend for blood work and a urinalysis. There's something causing her weight loss and diarrhea, and it could be a digestive issue. I think at this point, further testing is needed. I hope this helps!

It would be worth rechecking a stool sample to be sure the giardia is cleared and also to check for signs of poor digestion. German shepherds are prone to very sensitive digestion and in some cases are unable to digest regular food due to an enzyme deficiency from the pancreas. In these cases, diet change to a hydrolyzed food and enzyme supplementation can be very helpful to restore body weight

If she is also urinating a lot and is underweight I’d recommend a vet exam, blood work and fecal testing. I’d assume at her young age the drinking excessively is behavioral. If she isn’t active and not in the heat limit her water intake a bit. As for her appetite if she is underweight then I’d be concerned for parasites or a digestive disorder. Shepherds can be affected by pancreatic insufficiency and inflammatory bowel disease. If she is underweight then increase her food by 25% every 2-4 weeks until she had has ideal body condition.
