Why is my old unneutered dog skinny with bloody penis discharge?

I would recommend taking him to a vet for an exam - they will probably want to collect a urine sample for testing, check his prostate during a rectal exam, and examine his penis closely outside the sheath to look for any problems. From there, x-rays and ultrasound can be used to look at the prostate, if needed. Ideally, he should be examined by his veterinarian so that they can evaluate his penis, prepuce and look for evidence of stones or a UTI. Your vet can check a urine sample and take an xray to evaluate his bladder and prostate. Without doing diagnostic testing, it is impossible to know the exact cause and best way to treat.

This is probably a prostate disease (cancer or infection), this is common in unneautered older dogs, the vet will check the prostate, if it is enlarged he will need to have an ultrasound or CT done to find out if it is a cancer or an infection, he will need to be neutered ASAP.
