Old dog with anal mass & Cushing's: Is surgery safe for her?

Eighteen years is quite old for a dog, so I understand why you may be hesitant to go through any procedures that may decrease her quality of life. At this age it's uncertain how much longer she will live (with or without the tumor) and it seems like she may have other problems,or possible spread of the tumor that would not give a favorable prognosis for recovering from surgery. I would definitely want to solve the problem of not eating before recommending surgery for the tumor. It was helpful to see the notes and lab work, but I would need to further evaluate her and discuss this more in order to give you some more helpful advice. I think it would be worthwhile to discuss this with your veterinarian again and ask what they would do if it were their dog. This may help you with your decision. Gumbo definitely is at increased anesthetic risk (risk of adverse events during anesthesia and risk of poor recovery) at age 14 (7-8 years of age is considered geriatric), even with normal blood work (normal liver/kidney values). In addition those are extremely large masses and surgical removal could definitely affect anal tone (result in incontinence) assuming that anesthesia and recovery do go well. There is also a possibility that the type of cancer Gumbo has is metastatic and has already spread to the lungs for which the prognosis is poor (chest rads would be recommended prior to proceeding with surgery). I would recommend bringing Gumbo to have the anal glands checked regularly to make sure that they are empty. I also recommend keeping Gumbo as comfortable as possible as consulting with your veterinarian about treatment and humane endpoints if the masses become ulcerated.

If it is possible, I would recommend having the x-rays done without sedation and depending on how the x-rays look and how her blood work looks, consider removal of the mass under anesthesia. She is not too old for anesthesia but the risk of anesthetic complications rises with age. As long as her heart, lungs and blood work are normal, she should be fine under anesthesia. I am so sorry that you are exeoeriencing these problems with Wiggles I am sure it's very upsetting. Because mammary tumours are Hormone linked I would advise spaying her when she has the tumours removed. The last thing you would want is for Wiggles to have another mammary tumour in the future and therefore another surgery. For anaesthetics for older dogs many precautions are taken. Your veterinarian will discuss these with you to help put your mind at ease. They include blood pressure monitoring and intravenous fluid treatment before, during and after the surgery. Yes, it is most likely mammary (breast) cancer. Very common to happen in dogs that are not spayed. That is why we recommended spaying dogs early in life. 1. I suggest you find a vet that will do both surgeries at once. There is no sense in doing two surgeries a month apart. They can both be done safely at the same time in most cases. She may be one of those cases that the mass is extensive and that is why your vet does not want to do them at the same time. I don't know since I have not examined her myself but in most cases I would do both surgeries together. Just my opinion. 2. You need to get her spayed, even at 8 years old. If you don't, then she may end up with more mammary tumors or a pyometra or she may pass away. Yes, a blood test before the spay is indicated absolutely. 3. It should not change her much. If you don't do it then she can certainly die just like a person with breast cancer so you really have no option but to have it done. Hope this helps. Best wishes.

It sounds as if your vet is concerned that your dog has a malignant tumor, most likely squamous cell carcinoma. If there is suspicion about the malignancy of a tumor the best course of action is to have it removed. I can understand your concerns about Dahlia undergoing surgery but it would be safer and easier to remove it now, while it is smaller, then it would be if it was left to continue to grow. If your vet recommends removing the growth, it's likely a good idea. In a senior dog like Cookie, there is always a chance it could be cancer, which is why it's ideal to remove it to prevent it from spreading. If the growth is cut out with wide margins and the cancer hasn't already spread, it is possible that it can be cured completely without further treatment. The vet will do everything they can to remove the tumor fully, and I would imagine they would let you know if there was a concern that it couldn't be removed completely or if she wouldn't heal properly. I do advocate for removing growths in a senior dog especially if there is a concern it's cancer, but I highly recommend speaking to your vet about your concerns. Having an in-depth conversation with them may help allay your worries. I hope this helps!
