Can my 18-year-old dog survive breast tumor removal surgery?

Age is not necessarily a contraindication to surgery if they are being careful with the anesthesia. Keep in mind that most dogs that develop mammary tumors are older and have surgery and do fine. Unfortunately, this is a difficult situation because most tumors that are cancerous and rupture are not going to heal on their own. A double mastectomy will be a very large and could be difficult recovery time for an 11 year old dog. Also, please know that even with surgery, you may not get another 3 years out of her simply because she is already at the top end of her life span. Also, there is always the risk that the cancer has already spread. Another option you may want to talk to your vet about is whether they can just do a palliative "debulking" of the mass that has ruptured. If there is a way to just resect the one tumor that has ruptured, this potentially could give her a good quality of life without doing a full double mastectomy. With the tumor being ruptured, I am afraid you are being forced to either do a debulking surgery, full mastectomy, or euthanasia. Leaving it alone isn't a good option because it won't likely heal on its own and if it isn't already, it will become infected.

Assuming Kara is in good health otherwise - which should be established with blood work and x-rays of the chest, to ensure that the tumor hasn't metastasized - then I would recommend proceeding with surgery. Mammary tumors can be quite invasive and aggressive, but depending on the grade surgery can be curative. Just because a dog is old, it doesn't necessarily mean they can't have anesthesia - you just have to take precautions, and find a vet who is capable of taking special care. I would also get a definitive diagnosis as to what the mass is before making any decisions, There are lots of benign lumps that grow all over dogs' bodies, and just because this one is in the mammary area, it isn't necessarily an aggressive cancer. A fine needle aspiration biopsy can usually tell you what these are.

I am so sorry that you are exeoeriencing these problems with Wiggles I am sure it's very upsetting. Because mammary tumours are Hormone linked I would advise spaying her when she has the tumours removed. The last thing you would want is for Wiggles to have another mammary tumour in the future and therefore another surgery. For anaesthetics for older dogs many precautions are taken. Your veterinarian will discuss these with you to help put your mind at ease. They include blood pressure monitoring and intravenous fluid treatment before, during and after the surgery. If it is possible, I would recommend having the x-rays done without sedation and depending on how the x-rays look and how her blood work looks, consider removal of the mass under anesthesia. She is not too old for anesthesia but the risk of anesthetic complications rises with age. As long as her heart, lungs and blood work are normal, she should be fine under anesthesia. Gumbo definitely is at increased anesthetic risk (risk of adverse events during anesthesia and risk of poor recovery) at age 14 (7-8 years of age is considered geriatric), even with normal blood work (normal liver/kidney values). In addition those are extremely large masses and surgical removal could definitely affect anal tone (result in incontinence) assuming that anesthesia and recovery do go well. There is also a possibility that the type of cancer Gumbo has is metastatic and has already spread to the lungs for which the prognosis is poor (chest rads would be recommended prior to proceeding with surgery). I would recommend bringing Gumbo to have the anal glands checked regularly to make sure that they are empty. I also recommend keeping Gumbo as comfortable as possible as consulting with your veterinarian about treatment and humane endpoints if the masses become ulcerated.

The next step before considering surgery would be proceeding with a CT scan to check on how invasive the tumor appears to be. Then the next treatment option can be determined. A FNA can be performed at the time to determine the tumor type. If not too invasive surgery may be possible. If too invasive, and this is a thyroid carcinoma (the most common tumor in that region), radiation therapy or chemotherapy may be next best options. The prognosis with these tumors depends on the type which is why that needs to be tested. Also, it depends on how invasive it is and if it can be resected.
