Dog worse after vet visit, lethargic. Should I get a second opinion?

Unfortunately, bloodwork will only tell us part of what is going on internally. Sometimes figuring out what is wrong can require additional testing such as urine samples, x-rays or ultrasounds. You didn't mention Luna's age, but have her listed as unspayed. Depending on when her last heat cycle was, you may want to discus with your veterinarian if she could have been bred or is having a uterine infection. Not eating or drinking is a serious medical problem, so I would strongly recommend contacting your veterinarian for their recommendation on the next diagnostic or treatment steps you can take with Luna to figure out what is going on and how to make her feel better. I recommend having tests done on Luna such as blood work, a urinalysis, and possibly x-rays. This can help them diagnose what is going on with her. I would take her back into a vet today, poor girl.

Without the exact information, I can only assume that she had to have an emergency spay because of a serious problem like a pyometra, difficult birth, etc. Just a routine spay is still a major procedure, but an emergency spay because of the above conditions, is even more significant. Although it is possible, I am quite hesitant to think that this nipple redness would cause her to act so lethargic and unenthused about going outside. I am more likely to believe that there is some internal issue like pain or infection that is causing her to display this behavior. So yes, because of the above reasoning, I truly believe you should have her rechecked by your veterinarian as soon as possible to make sure everything is ok. Rechecking lab values will be helpful to see if there are any changes, and IV fluids would be helpful to fight inflammation and dehydration. It's a little difficult to see for certain in the picture, but I do see a fluid filled structure in the photo near the urinary bladder that certainly could be the uterus. In your final picture the discharge on the ground does not look what I would typically expect from vaginitis. You may want to get a third opinion from a veterinarian if you're concerned about going forward with surgery. If it is a uterine infection it is unlikely to clear with antibiotics alone and even if it does the infection returns with the next heat cycle, so I typically recommend surgery whenever I am concerned about a pyometra.

Yes! If there is no improvement then he definitely needs a recheck. They can always aspirate or biopsy a sample of that growth to determine if it is a lymph node or a salivary gland because it may need a surgery ir simply stronger antibiotics. Blood work may not be a bad idea either to make sure there isn't a systemic infection.

This sounds like a pretty complex situation, and I'd agree that an infection of some sort seems likely with a high WBC. I'd be worried about something viral, perhaps even meningitis, especially if it seems that she has neck pain, but that's basically a guess on my part. She should be tested for tick-borne diseases (Rocky Mountain Spotted Fever, ehrlichia, babesia, Lyme) if that hasn't been done. I'm not sure how far you are from a specialty hospital, but it sounds like she's still not out of the woods, and in the absence of a definitive diagnosis, I too would be worried. I would recommend taking Sky to a specialty hospital, where they might be more capable of diagnosing the problem and getting her the appropriate treatment.
