My dog is vomiting constantly and not eating after vet visit. What's next?

My dog is very ill . He has vomited 8 times in last 24 hrs . He first had this kind of poop , Picture attached. He was eating fine, Vet gave him an injection for stomach infection and deworming . He was eating fine than . This was 72 hours ago . 48 hours ago he stopped eating and than vomiting started . After that he has been vomiting. The vet gave him some drips flagyl and risek . She has said not to give him any thing to eat for 48 hrs . I need a second advice

The bilirubin is a bit high, as is the ALP (alkaline phosphatase) but neither of these values is super specific in terms of what the cause or the best treatment is. We can see the ALP be elevated with stress, age, and sometimes even normal cell regeneration within the liver. The bilirubin can be slightly elevated when the blood sample is what we call "hemolyzed," which can sometimes happen when blood is spun down for a blood test to be run.
Therefore, if he is still showing symptoms of illness, I would echo what Dr Ana said; it may be time to consider imaging. Either X-rays or an abdominal ultrasound maybe helpful in identifying the problem and therefore identifying the best way to treat it. Take him back to the vet if he's still having troubles, as he probably is in need of some more fluids as well.

    Thank you for submitting your question regarding your dog. I recommend that he has further testing. X-rays of the belly are warranted in cases of frequent vomiting. Fecal testing may be necessary to look for intestinal parasites, bacterial or even a viral infection. If he is not current on his vaccines, parvovirus is a possibility. 48 hours is a long time to fast a dog unless they are recieving continuous IV fluids. Medication should be given to make him less nauseous and control the vomiting so that he will eat sooner. I hope this information helps. Good luck!

It sounds like their treatment didn’t resolve the symptoms. Things like pancreatitis or GI obstruction won’t show up on routine blood work, so I would recommend calling the vet today for what needs to happens next. If an obstruction is one of the vet’s concerns, they may recommend x-rays or ultrasound. If pancreatitis is a concern, then usually they need to go on IV fluids for a day or two.

The next thing to consider would be an abdominal ultrasound and potentially considering gastroscopy (endoscopic examination of the upper GI tract). A barium study might also be useful when doing x-rays to see what the barium does as it moves through the GI tract. I hope this helps!

Why is my dog still vomiting and not eating after clear X-rays?
Acute pancreatitis and gastrointestinal inflammation/infections are still possible as well as foreign body ingestion with material not visible on X rays (ultrasound in these cases might be more sensitive). Given the lack of improvement, I would suggest to have her checked again and have a test for pancreatitis (it is a blood test called cPLI) performed. If not eating and probably in a sort of abdominal discomfort, nutritional support and pain relief meds are needed.

Mild increases in liver values can be seen for a number of different reasons, and sometimes it can be very difficult to determine what the underlying cause might be. Infections like leptospirosis, migrating parasites, congenital diseases like a portosystemic shunt or some toxins can cause liver values to increase. It seems like Leo is on supportive medications for the liver - it would be helpful to know if his values are increasing or decreasing since trends are most important. An abdominal ultrasound might be the next step, or you could also ask about referral to a specialist if available in your area.

BUN and CREA are severely elevated and the cause of it should be ideally understood by performing a urine analysis (and specifically a urine specific gravity to rule out kidney failure). Mild anemia (lower RBCs) can be present during kidney failure. Amylase is another important value as severe elevation could be compatible with pancreatitis, which can cause the signs you are describing. CPLI test can be performed as well to rule out pancreatitis. Please discuss all the options with your local vet.

Why is my dog vomiting water with high liver and kidney levels?
Hello, thank you for your question about Loki. ALT is a liver enzyme and elevations can be a sign of underlying liver disease (caused by infection, inflammation, etc). BUN is considered a marker of kidney function but can also be elevated for other reasons, such as GI bleeding. Depending on the severity of these elevations and Loki's history I would recommend testing him for an infection called leptospirosis, as this disease can cause liver and kidney abnormalities. Alternatively, If Loki is still feeling well and the metronidazole is improving his symptoms, you could continue with this treatment regimen and monitor him for development of any symptoms. I would recommend rechecking these values after he has finished the course of metronidazole and considering the need for other diagnostics at that time. Hope this helps, good luck!
