My puppy has skin wounds. Is it ticks, fleas, or mange? What's causing it?

Unfortunately fleas do not live on the dog, but only hop on to feed and then they hop off and stay in his environment (your apartment!) until it is time to feed again. So you need to treat all pets in the household as well as the environment (which contains the fleas, their eggs and larvae). Flea treatment:  1) Get some flea medication such as a Flea spray or a spot on from a pet shop or your veterinarian and apply monthly (which you have done already) 2) get flea powder, put some in your vacuum cleaner bag and vacuum your house and furniture and also non-carpeted areas frequently to remove flea larvae and eggs which live off house dust., Dispose of the bag in the trash outside. Hot-wash all the pet's bedding, If you do this diligently for a few months you will get rid of the fleas. It really depends on what he has. Lice and ticks are very different things. I would recommend starting with an oral flea/tick preventive (which you'll need to get from his vet) and a good bath with general dog shampoo. Actually, given his tender age, starting with a vet visit would be best so they can treat him appropriately from the get go.

Skin parasites can range from ticks/fleas where I would suggest a flea/tick preventive such as Seresto collar, Capstar or Bravecto. Bravecto is an oral medication that off-label can also protect from mange and mites. Bravecto can be ordered by your vet and the collar or topical flea/tick preventive is over the counter in the USA. Poor puppy! You need to see the veterinarian. Over the counter products are highly unlikely to kill the mites. Additionally, if he has Scabies, this will infest humans for a temporary time period. Additionally, ringworm can also look like this (which humans can also get). The veterinarian needs to do a skin scrape which will detect if mites are present. Treatment will be based upon results of diagnostics. He will likely get a special shampoo and antibiotics as well as medications to treat to primary cause.
