My dog is sick and won't eat for 20 days. What should I do next?

That duration of inappetance is very concerning. Did you family vet prescribe some therapy, anti nausea medication? It sounds like your kid needs to see a specialist. For a case like this an internal medicine specialist would be the best person to see. A full abdominal ultrasound with an experienced ultrasonographer (either a radiologist or internist) is likely the first step if blood work and xrays are normal. Unfortunately some pets cases are complex and some disease processes require more advanced testing then what family vets can provide. Likely your family vet has ruled out some common things but an internist will look for the uncommon things. Unfortunately decreased appetite is a very non specific symptom because in animals when they do not feel good for any reason they can stop eating. I would recommend starting with some testing (bloodwork, urinalysis, x-rays) through your vet to determine what is causing this so that we know how best to help your sweet girl. Once we know the cause, proper medical treatment can help stabilize her condition. Until you can get her in try offering her some Pedialyte and/or Pediasure to provide some fluid support and nutritional support until she can get seen. Sasher does sound pretty sick if she hasn't eaten for three days, and unfortunately anorexia is a general sign of many different illnesses. Things that could be causing this include dental disease, liver failure, kidney failure, heart disease, systemic illness, injury, or cancer to name some. The first thing to do is get her in to see the vet to find out if there is some kind of underlying illness that is causing her to lose her appetite. She may have an allergy, some kind of painful condition that makes her not want to eat, a metabolic disorder, there are any number of possibilities. I recommend getting her checked by your vet as soon as possible for an exam and labwork,even x-rays if the other tests are inconclusive. Based on these results your vet will be better able to tell you what's going on and how best to treat it.

If I'm reading this correctly, the last time Nypsy ate was a few days before October 3rd, which means she has not eaten in about two weeks.
