Why is my dog scratching his ears until they bleed and scab?

This presentation could be due to a trauma or localized spot of skin infection (likely to be caused by bacteria or fungal). A check up, appropriate disinfection and possibly antibiotics or antifungal meds (oral, topical or in a shampoo formulation) are needed to solve the problem. From what i can see in the picture it looks badly infected, this means that Bora must have an antibiotic course in order for it to heal, you should return to the vet in order to get it treated, without antibiotics this condition will not get better.

You may want to give your vet a call and tell them it is getting worse. It sounds like this scab could be a hot spot or superficial bacterial infection. While stopping the itching sometimes helps an infection resolve (if that’s what this is) other times topical or oral antibiotics are needed. He could have an infection and/or allergic dermatitis. Since this has been going on for a couple years, he should be examined by a vet to determine the cause and treat. He may need an antibiotic, anti-histamine, and/or anti-inflammatory to treat. He may need to stay on an anti-histamine long term if it is related to allergies.

I would suspect that Augie has an ear infection leading him to itching and rub his eat against the carpet, thus causing the scab you now see. I would take him into the vet today for this problem. Ear infections are caused by ear mites, yeast, or bacteria in dogs, and the cause of the ear infection needs to be diagnosed so he can be prescribed the correct medication. While at the vet, they can treat the scab as well. I would refrain from putting Neosprin on it, you don't want any to get into his ear, it's starting to heal if there is a scab, and it won't do anything to help him if he does have an ear infection. It looks like what we call a hot spot or moist dermatitis. These looks really painful. I would have him seen. He will most likely need some antibiotics and some topical medication to help dry this up. They can also give you something for the itching or pain relief. These usually heal pretty quickly once they stop messing with them.
