Why is my small dog reverse sneezing, vomiting, and hoarse?

I'm glad to hear that Sassy seems to be doing better; do I understand that correctly? It is possible that she may have been having some gastritis or an upset stomach that caused the vomiting; and perhaps some gastric reflux. If they regurgitate and the stomach contents come up to the back of the throat from reflux, that could potentially lead to some reverse sneezing and irritation in the throat. In a more serious scenario, aspiration can occur (the stomach contents from vomit or regurgitation go down into the airways) which could lead to pneumonia. You can ask your vet if an acid reducer for her stomach may be warranted, but other than that, as long as she's doing well I don't think you need to worry.

You can try to prevent the episodes by minimizing her level of excitement (e.g. Not getting overly excited yourself when she greets you at the door), and using a harness instead of a collar when out for walks. One thing that I did with my dog to stop the reverse sneezing episodes was to gently put my finger across his nostrils, to block the airflow through the nose and force him to breathe through his mouth. This worked so well that he eventually learned to do it himself! If the reverse sneezing and coughing episodes seem to be truly bothersome to Lulu, I would recommend taking her to the vet. I'm wondering if Luffy's problem while breathing might be a reverse sneeze. The attempting to vomit may actually be a non-productive cough, the two can look very similar. With either of these problems a physical exam with a veterinarian could be completely normal. If you're able to take a video of what's going on you might be able to show your veterinarian during a recheck visit which might help them determine what Luffy is doing.

A cough like the one you're describing in a little dog like Lace, I'd be worried about a collapsing trachea or congestive heart failure - both conditions are very common in little dogs and can cause dramatic coughs. Other possibilities would be kennel cough, pneumonia, allergies, and parasites. Hairballs are pretty uncommon in dogs, though bronchitis is possible if this were an allergic issue - but allergic coughs usually start at a younger age.
