My dog's red skin marks are spreading after using a cream. What to do?

Those look like pyoderma/bacterial skin infection erythematous (red) collarettes. The treatment is typically a 10 day course of prescription Simplicef (cefpodoxime) oral antibiotic, a medicated antibacterial/antifungal shampoo such as Malaseb (available on chewy.com or petco.com) every 2-3 days for 2 weeks and a topical prescription antibiotic/steroid ointment such as Dermavet. Your vet will likely be able to dispense all 3 after an exam if it is confirmed to a bacterial skin infection. Fungal infections are zoonotic/transmissible to humans and typically present as circular areas of complete hair loss that are often scaly. Good luck!

Oh poor Doja! You have got to get her to stop scratching it. She may need a bigger E-collar. I think it would be best to take her back to your veterinarian and discuss skin scraping, cytology and a culture. She may have a fungus or resistant bacterial infection which would not respond to the prescribed medications.

It does look like a skin infection but the origin (bacterial, fungal, yeast) I don't know just by looking at it. If it is bacterial, sometimes oral antibiotics are needed if topical therapy is not working. I don't know what cream you are using as I am not aware of a chlorhexidine cream, or how long you have been using it, so it's hard for me to comment much more on what you are already doing. I personally like these chlorhexidine wipes because they also contain an anti-fungal/anti-yeast component (climabazole) and a component to help repair the skin (phytosphingosine) so it is gentle. So, if she is licking, you will need to get an E-collar (cone) for her to wear until this is healed. So, if you want to try the medicated pads, I would do that. If there is still not resolution after a few weeks then she should go back to the vet. Skin maladies typically take quite a long time to respond completely to treatment so as long as it's not bothering her too much just continue on for a few more weeks. It is likely dermatitis, ie bacterial infection, as the skin may be irritated from the shaving and the hotspot likely was infected with bacteria. Steroid ointment will not help, but rather make it worse. Your dog will likely need prescription antibiotics from your veterinarian after an exam and cleaning and disinfecting the site.

It’s uncommon for older dogs to get ringworm, and when they do it’s generally on the face. This area may have started to itch and she got it red and irritated from licking. I would clean the area with a medicated shampoo that kills both bacteria and yeast. Then apply a thin film of antibacterial ointment on it. In addition use a cone if she’s licking the area. If you wash your hands well after touching her you shouldn’t get anything. If it’s not healing or there are other areas then please take her in for an exam and skin tests to determine if it’s ringworm or infection. Circular peeling areas might be an indication of ring worm, a fungal infection that can be spread to other animals as well as people. The most common treatment is an antifungal drug called clotrimazole that is used topically. It is available in sprays and creams. Treat the area carefully and use gloves as not to risk spreading the infection elsewhere. Formulations are also available through your veterinarian. The vet would be able to obtain samples from the affected region to determine if ring worm is in fact the cause of what you are seeing.

It looks as if your dog has a skin infection, most commonly caused by an allergy, a skin mite (Demodex) or fleas. 

The allergy could be a due to a protein in your dog’s food or an environmental allergy like to dust, pollens, grasses, etc. 

The allergy causes your dog to itch and chew at the skin which breaks down the skin barrier. This then leads to infection. Skin infections are typically characterized by large areas of hair loss, redness, crusting, flaking, and pustules. A thorough skin examination by your veterinarian should be able to determine the cause of the itch so it can be treated appropriately. Your dog will most likely need antibiotics to help clear up the infection and possibly an allergy/anti-itch medication like Apoquel, Atopica, or Cytopoint to give some relief while the infection clears up. A medicated shampoo may be beneficial as well. I would also recommend he be on a good flea control routinely too. Once the infection has resolved a long term allergy medication (like Apoquel or Cytopoint) can be given on a regular basis to prevent any future problems. I hope this helps.
