My dog's rash is worse, biting hip. Vet spray failed. What to do?

From the excellent picture you have provided it looks like Winstyn may have moist dermatitis, or a "hot spot". Unfortunately, hot spots often require a trip to the vet for clipping of the area, cleaning, and prescription of a healing spray / ointment with antibiotics and a steroid. All of these interventions will help the area heal quickly, and minimise the level of discomfort that Winstyn has to experience. Due to the location of the sore it will particularly difficult to treat at home. In the meantime you can help Winstyn by; Placing a an e-collar to prevent him biting or attracting at the sore and making it worse Attempt to air out the area as much as possible to help it dry out. I'm sorry to hear that Snoopy is having skin problems. He is definitely itchy if he is biting at the affected area. I would recommend that he sees his veterinarian. He will likely need a medication to decrease the itching and possibly an antibiotic. I typically send home a medicated shampoo as well. The bigger question is why he is itching in the first place. He could have an allergy to something in his environment, diet or even fleas. His veterinarian may recommend testing for mites or ringworm as well. In the meantime, you can try bathing him with a moisturizing shampoo and conditioner such as an oatmeal or fatty acid based. He should wear an E-collar as well to prevent further self trauma. This looks like a moist bacterial dermatitis. This likely happened because the joint hurt and your dog was licking the area. This damaged the skin and bacteria can then cause the dermatitis. You will need to take Ranger to a veterinarian for an exam of the hip and the skin. Ranger will likely need pain medication for the pain in his hip and a prescription antibiotic for the dermatitis. The are of the affected skin will need to be shaved, cleaned and disinfected.

It does look like a skin infection but the origin (bacterial, fungal, yeast) I don't know just by looking at it. If it is bacterial, sometimes oral antibiotics are needed if topical therapy is not working. I don't know what cream you are using as I am not aware of a chlorhexidine cream, or how long you have been using it, so it's hard for me to comment much more on what you are already doing. I personally like these chlorhexidine wipes because they also contain an anti-fungal/anti-yeast component (climabazole) and a component to help repair the skin (phytosphingosine) so it is gentle. So, if she is licking, you will need to get an E-collar (cone) for her to wear until this is healed. So, if you want to try the medicated pads, I would do that. If there is still not resolution after a few weeks then she should go back to the vet. Skin maladies typically take quite a long time to respond completely to treatment so as long as it's not bothering her too much just continue on for a few more weeks. Without an accurate diagnosis it is impossible to guess the most appropriate management for any skin condition. I would recommend a sedated exam if he is uncomfortable being handled in order to confirm or rule out causes such as infection, allergy, mites etc as each would require different treatment. If you feel he is food sensitive then a hydrolysed diet such as royal canin hypoallergenic should be trialled for 8-12 weeks (no treats, snacks or flavoured tablets to be fed during this time). Depending on the diagnosis, fatty acid supplements, medicated shampoos, allergy medications and flea/mite prevention may also be required to ease the constant discomfort and irritation
