My dog has a rash and bald spot not healing with cream. What is it?

Those spots on Maybelle could be several things including allergies, skin mites, flea bites, ringworm (which humans can get) or a bacterial infection. You can try adding in an omega 3 fatty acid supplement for dogs, this can help improve the skin health. You may also try changing her diet to something like Royal Canin or Nutro as they are aimed at skin health. You can wash the lesions with soap once a day and rinse well (don't wash her whole body, just those spots). Protect the areas from licking or scratching. If they don't start looking better within about a week or if they get worse, have her examined by the veterinarian who may prescribe medications. Ouch, poor Wan! Skin rashes can be caused by a number of things including bacterial infections, fungus, yeast, metabolic issues, food and environmental allergies and more. If the cream you're using isn't working, it is a good idea to bring Wan back to the vet for another exam. Your vet may recommend a skin scraping of the affected area to figure out the underlying cause, or may recommend blood work to check for metabolic issues causing the skin changes. From there, your vet can prescribe additional medications or medicated shampoos. Keeping Wan from licking and chewing the area with an Elizabethan (cone) collar is best to prevent further irritation.

It is very possible that Merlin has or had fleas on him. If that was the case, an untreated allergy can last 30 days. If he has been on a flea prevention product, we would need to rule out a microscopic mite, called demodex, or a fungal infection, called Ringworm. If it has been getting bigger over the last few weeks, you should really take Milo to your local vets to get the bald patch examined and start treatment. Most likely, the treatment will be in form of antibacterial/antifungal cream or ointment.

Unfortunately, we are legally prohibited from recommending such a medication in the absence of a physical exam and a VPCR (Veterinarian-Patient-Client relationship). If current therapy has not helped, consider a recheck with your veterinarian. A biopsy of the area or consult with a veterinary dermatologist may be recommended.

I am suspicious that this could be a lesion caused by either demodex mite or ringworm. I would highly encourage you to have a follow up appointment with your veterinarian. A skin scrape can be done to look for mites, they may consider a trial anti mite therapy in addition to an a topical anti fungal medication. It does look like a skin infection but the origin (bacterial, fungal, yeast) I don't know just by looking at it. If it is bacterial, sometimes oral antibiotics are needed if topical therapy is not working. I don't know what cream you are using as I am not aware of a chlorhexidine cream, or how long you have been using it, so it's hard for me to comment much more on what you are already doing. I personally like these chlorhexidine wipes because they also contain an anti-fungal/anti-yeast component (climabazole) and a component to help repair the skin (phytosphingosine) so it is gentle. So, if she is licking, you will need to get an E-collar (cone) for her to wear until this is healed. So, if you want to try the medicated pads, I would do that. If there is still not resolution after a few weeks then she should go back to the vet. Skin maladies typically take quite a long time to respond completely to treatment so as long as it's not bothering her too much just continue on for a few more weeks.
