That is a really large swelling so he should be examined and treated by a vet. Your vet can do some testing to figure out the cause and then prescribe medications to treat. He may need an antibiotic, anti-histamine, corticosteroid, or other medications. Please call your vet and have him examined.

Poor baby. Definitely looks red and irritated. It may need oral antibiotics or possibly just some topical antibiotics. I would call and schedule an exam and get the correct medications. So, there are a number of things this could be. 1) This could be an allergic response to something in her environment or an insect, 2) This could be a fungal or bacterial infection that could be primary OR secondary to trauma or an allergy. There is some evidence of swelling and bruising, but it doesn't appear to be badly infected. I would also encourage you to get her an e-collar to prevent her from causing any self-inflicted trauma, thus re-aggravating the lesion. There are also videos online on how to make homemade ones, though many times these are not as effective or will not hold up for very long in young, rambunctious dogs. If you are worried about possible infection, check the area for heat and take your dog''s temperature (anything above about 103 would be concerning for a dog sitting comfortably at home). If all of those came back negative, the next step is to treat the allergy symptomatically and try to discover the cause of the allergy. Three main type of allergies are usually affecting pets: food allergies, flea and parasite related ones and environmental type (allergens coming from trees, pollens, dust etc etc). That does look quite irritated in the picture. Keiko could have an upper respiratory infection causing this or even a foreign body.
