Yes, you can use an unscented sanitary pad in a doggie diaper. You may ask your veterinarian if changing medications may help to urine leakage. There are a couple products (incurin and Proin) that tend to work well but with the heart enlargement and arrhythmia, your vet will need to decide if these medications are worthwhile. You may also look for a water proof cover to keep over the bed to at least decrease the mess. Tampons can easily cause obstructions in the GI tract, especially since they are absorbent and can get large in the stomach from liquid in there. if Bella ate a tampon, you need to take her into a vet immediately to get it removed. If she is on heat, no you cannot use a tampon to control her bleeding. It's not safe to use at all, and are not meant for dogs. You can put a diaper on her to catch any bloody discharge. Some dogs have a decreased appetite and are lethargic while others are hyperactive. There are no need for medications as they shouldn't be painful. Just always have her on a leash and do not let her outside a lone or around intact males dogs. I would have her spayed 2 months after the bleeding stops. Any pet-store will have diaper products for female dogs in heat, so you can go by and get some. Usually small breed dogs, especially on their first heat don't need any diapers and won't spot too much, and usually will clean themselves up. I recommend that you wait about 1-2 months and then have Lulu spayed before her next heat cycle. Spaying prevents the common and severe condition of pyometra, and also prevents unwanted pregnancies (shih-tzus often have trouble with birth and often require a c-section and get eclampsia, so you should not breed her especially if she is your first dog), and decreases her risks of being hit by a car (less roaming) and also decreases the risks of many cancers. Doggy diapers are definitely a good way to keep from having to clean-up blood all day. The cone may be necessary if she tries to take off the diaper, but some dogs tolerate the diaper well. Keeping her outside is another option if the weather is OK, but make sure you have a good fence to keep out any possible intact males in the neighborhood...they will smell her from a mile away.
