My dog accidentally took extra medicine. Should I call the vet?

With Crosby's size and the amount he was given, you should not seen any major side effects apart from maybe vomiting / diarrhea. You may see some vomiting and/or diarrhea. More serious toxicity is not a concern with a double dose of this antibiotic. Fast Milo for 24 hours then feed a bland diet in small, frequent amounts until symptoms resolve if necessary.

Don' t worry about having given her a one off overdose. It is a third generation cephalosporin antibiotic that it's quite often used at high dosages for skin problems. If given continuously at double dosage, I would recommend to check her liver through a blood test but since it just occurred once I would not worry on this occasion.

The dose range of metronidazole can go up to 11mg per day, which would be 550mg at her weight. Depending on the infection, the dose rate can be even higher than 11mg. If it was Heartgard, there is no cause for concern. It is an extremely safe product and very difficult to overdose. If it was a different preparation of ivermectin, a double dose may not be cause for concern unless your dog is sensitive to it or carries the MDR1 gene. Typically, a non sensitive dog requires dosing around 2000mcg/kg before clinical signs become apparent. See your vet for an exam as soon as you can if you are concerned. Bring the container of ivermectin with you. Treatment for ivermectin toxicity is supportive in nature. Monitor for vomiting, excess salivation, ataxia (wobbly gait), dilated pupils or tremors.

You did not mess up, its ok! They are both for fleas but work in different ways. Nexgard will kill the adult fleas (as well as ticks). However adult fleas are only a small part of the problem when it comes to fleas, because the eggs that may have been laid by the adult fleas will sprinkle off your dog, hatch in your carpet (eww I know) and the baby larvae will find their way back onto your dog (or other pets, or even you in very severe infestations).
